Cuidados paliativos ao paciente com HIV/AIDS: uma abordagem bioética.

Resumo: INTRODUCTION: Palliative care is the active and total patient whose disease no longer responds to curative treatment, have great importance in the care of patients with chronic, progressive, advanced, degenerative, incurable disease or end stage such Immunodeficiency Syndrome Acquired AIDS. OBJECTIVES: This study was to thread the following objectives: to investigate the understanding of nurses about the goals of palliative care; to identify the ethical observances adopted by nurses in promoting palliative care to patients with HIV / AIDS. METHODOLOGY: This is an exploratory qualitative approach. Regarding the scenario of the research, it was performed in the Clinic of Infectious and Contagious Diseases (DIC) and the Maternal and Child Care Service (NCS-MI), University Hospital Lauro Wanderley (HULW), located in the city of João Pessoa. Participants were eighteen nurses; to enable data collection, we used a form containing questions relevant to the proposed objectives for the research. Data collection occurred from October to December 2012. DISCUSSION AND ANALYSIS OF DATA: The empirical study was analyzed qualitatively using the technique of content analysis, from the following phases: pre-analysis, material exploration, processing of results. The qualitative analysis yielded the following thematic categories: Category I: Purposes of Palliative Care: Improving the quality of life of patients and their families; Category II: Palliative Care targeted to patients with HIV / AIDS: ethical observances. The latter included two sub categories namely: bioethical principles, respect for human dignity, confidentiality and privacy. The results emphasized that the purpose of palliative care, according to the understanding of the nurses involved with the research was to improve the quality of life of patients and their families. It also pointed out that professionals recognize the importance of multidisciplinary and multi-professional team, to promote that type of care. Regarding ace ethical observances adopted by professionals stood out bioethics principles, respect for human dignity, confidentiality and privacy. These were considered of paramount importance to guide their care practice nurse within palliative care targeted to patients with HIV / AIDS. FINAL - It is hoped that this study will then help to subsidize bioethical reflection regarding the practice of palliative care patients with HIV / AIDS. Also contributing to increase the quantum of publications to the scientific community and, especially, for nursing professionals who work in health care. Considering the small number of studies focused on the respective subject in the context of national literature.

Considering the small number of studies focused on the respective subject in the context of national literature.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESINTRODUÇÃO: Os Cuidados Paliativos são cuidados ativos e totais ao paciente cuja doença não responde mais ao tratamento curativo, têm grande importância na assistência em pacientes com doenças crônicas, progressivas, avançadas, degenerativas, incuráveis ou doenças em estágio final a exemplo da Síndrome da Imunodeficiência Adquirida, AIDS. OBJETIVOS: este estudo teve como fio condutor os seguintes objetivos: investigar a compreensão de enfermeiros acerca das finalidades dos Cuidados Paliativos; Identificar as observâncias éticas adotadas por enfermeiros na promoção de Cuidados Paliativos à pacientes com HIV/AIDS. METODOLOGIA: trata-se de uma pesquisa exploratória com abordagem qualitativa. Quanto ao cenário da pesquisa foi realizada na Clinica de Doenças Infecto-Contagiosas (DIC) e no Serviço de Assistência Materno Infantil (SAE-MI) do Hospital Universitário Lauro Wanderley (HULW), localizado na cidade de João Pessoa-PB. Participaram do estudo dezoito enfermeiros, para viabilizar a coleta de dados, utilizou-se um formulário contendo questões pertinentes aos objetivos propostos para a pesquisa. A coleta dos dados ocorreu de outubro a dezembro de 2012. ANALISE E DISCUSSÃO DOS DADOS: O material empírico do estudo foi analisado qualitativamente por meio da técnica de análise de conteúdo, a partir das seguintes fases: pré-análise; exploração do material; tratamento dos resultados. Da análise qualitativa emergiram as seguintes categorias temáticas: Categoria I: Finalidades dos Cuidados Paliativos: melhorar a qualidade de vida de pacientes e familiares; Categoria II: Cuidados Paliativos direcionados ao paciente com HIV/AIDS: observâncias éticas. Esta última contemplou duas sub categorias a saber: princípios da bioética; respeito a dignidade humana, sigilo e privacidade. Os resultados ressaltaram que as finalidades dos cuidados paliativos, de acordo com a compreensão dos enfermeiros evolvidos na investigação foi o de melhorar a qualidade de vida de pacientes e seus familiares. Apontaram também que os profissionais reconhecem a importância equipe multiprofissional e multidisciplinar para promoção da referida modalidade de cuidar. No que diz respeito ás observâncias éticas adotadas pelos profissionais destacou- se os princípios da bioética, respeito à dignidade humana, sigilo e privacidade. Estes foram considerados de suma importância para nortear a sua prática assistencial do enfermeiro no âmbito dos cuidados paliativos direcionada ao paciente com HIV/AIDS. CONSIDERAÇÕES FINAIS Espera-se então que este estudo venha contribuir para subsidiar a reflexão bioética no que concerne a prática dos cuidados paliativos pacientes com HIV/AIDS. Além disso, contribuir para ampliar o quântico de publicações para a comunidade cientifica e principalmente para os profissionais de enfermagem que atuam no âmbito assistencial.
