O impacto nos gastos com internações da população idosa, nas capitais brasileiras, em razão da migração da população idosa

Resumo: | The aging process of the Brazilian population is a phenomenon evidenced by several studies and is occurring quickly and intensely. Associated with this fact, the migratory dynamics can provoke the evolution of the population structure, intensifying or softening the aging process. The impact of this new demographic structure is significant, especially when we observe the problems associated with health care for the population and especially the elderly population, since they use the hospital services in a more intensive way than the other age groups, involving higher expenses with treatments prolonged and slower and more complicated recovery times. In this argument, the objective of this study is to analyze the impact on hospital expenses, agreed to the SUS, due to the migration of the elderly population in the Brazilian capitals selected for the study. It is in this panorama that the role of the health and the exact areas assumes special significance, so as to be structured a decision model that can be used in planning in the health area. The exploratory / descriptive and inferential study, developed from a quantitative approach, was carried out in selected Brazilian capitals for the study, according to inclusion criteria defined for capitals and for the main causes of hospital admissions of the elderly population. Statistical tools were also used for population projections, migrations, hospital admissions and hospitalization expenses. The results showed an epidemiological picture that portrays, for Brazilian health policy, the challenge of coping with old evils and not neglecting the problem of chronic diseases. In this context, cardiovascular diseases, neoplasias and chronic respiratory diseases play a decisive role in the expenditures of the Unified Health System. In this sense, the analysis of authorizations for hospital admissions (AIHs) has shown a tendency to increase in cases of hospitalizations and expenditures for the treatment of the elderly population according to the diseases identified with greater incidence in the records of the Unified Health System (SUS). When analyzing the migration of the elderly population in the capitals, which was part of this study, this was shown in a small proportion, so that the result of the statistical test confirmed that there is no significant change in the elderly population of each capital due to migration. That is, although migration is a confirmed phenomenon, it does not present enough values to impact the health care expenditures of the elderly population. However, it should be noted that the aging of the population is a relevant factor in SUS expenditures, since projections of population aging, number of visits and average values of AIHs show a generalized growth trend. |

Associado a esse fato, a dinâmica migratória pode provocar alteração na estrutura etária da população, intensificando ou amenizando o processo de envelhecimento. O impacto desta nova estrutura demográfica é significativo, principalmente quando se observa os problemas associados à atenção a saúde para a população e, principalmente, a população idosa, pois estes utilizam os serviços hospitalares de maneira mais intensiva que os demais grupos etários, envolvendo maiores gastos com tratamentos prolongados e de recuperação mais lenta e complicados. Nesse argumento, o objetivo deste trabalho é analisar o impacto nos gastos hospitalares, conveniados ao SUS, em função da migração da população idosa nas capitais brasileiras selecionadas para o estudo. É neste panorama que assume especial significado o papel das áreas da saúde e das exatas, de modo a ser estruturado um modelo de decisão que possa ser utilizado no planejamento na área da saúde. O estudo, do tipo exploratório/descritivo e inferencial, desenvolvido a partir de abordagem quantitativa foi realizado nas capitais brasileiras selecionadas, para o estudo, conforme os critérios de inclusão definidos para as capitais e para as principais causas de internações hospitalares da população idosa. Ainda foram utilizadas ferramentas estatísticas para projeções de população, migrações, internações hospitalares e gastos com internações. Os resultados demonstraram um quadro epidemiológico que retrata, para a política de saúde brasileira, o desafio do enfrentamento de males antigos e o não negligenciamento do problema das doenças crônicas. Nesse contexto, as doenças cardiovasculares, as neoplasias, as doenças respiratórias crônicas têm um papel decisivo nos gastos do Sistema Único de Saúde. Nesse sentido, a análise das autorizações de internações hospitalares (AIHs) demonstrou a tendência de aumento nos casos de internações e nos gastos para o tratamento da população idosa em função das doenças identificadas com maior incidência nos registros do Sistema Único de Saúde (SUS). Quando analisada a migração da população idosa nas capitais, que fizeram parte desse estudo, esta se mostrou numa proporção pequena, de tal forma que o resultado do teste estatístico confirmou que não existe uma alteração significativa na população idosa de cada capital em razão da migração. Ou seja, embora a migração seja um fenômeno confirmado, não apresenta valores suficientes para impactar nos gastos com os tratamentos de saúde da população idosa.
