Abandono ao tratamento: desafio para o controle da tuberculose

Resumo: | Tuberculosis remains standing out in the world, despite science allowed to get proper treatment and cure, with the support of the expansion strategy of Directly Observed Treatment, Short Course (DOTS) and decentralization of control to the Health Strategy Family, the disease has abandonment and non-adherence to treatment as major reasons for treatment failure, which leads to implications for public health and the health of the individual, its repercussions. Considering how the Family Health Team includes the actions for the control of tuberculosis and the care provided to patients and their families, this research aimed to analyze under the Primary Health Care, the factors related to noncompliance with treatment for tuberculosis in the patient's vision in the city of João Pessoa - PB, by size of family focus. This is an evaluative study of qualitative nature. The construction of the empirical material was collected through interviews, during October and November 2009, using oral history as methodological issue. We interviewed seven users who have had to abandon as a criterion of closure for the treatment of TB. After examining the material was produced one major category: Factors related to discontinuation of the treatment of tuberculosis according to the patient's perception of TB, from which emerged two empirical categories: Social vulnerability and abandonment of treatment of tuberculosis; The comprehensive care of the Health Team family for the patient with tuberculosis: strengths and weaknesses. The trajectory of the research revealed that the link with the Family Health Team is crucial to the success of treatment may compromise the continuity. We identified the need for the health care team know the project of happiness of the individual and his family, to better know and understand behaviors and values related to health-disease process. We also observed that the consultations are important strategies for patient monitoring and link building. It was found that it is imperative to involve the patient and his family in the care process, bringing to light the need for the practice of co-management and integral and humanized care, and encourage interdisciplinary partnerships and of intersectoral actions. The analysis of living conditions of the employees showed that all were in a situation of social vulnerability, noting that TB is linked to lower socioeconomic levels, which leads to reflection that the patient needs care perceived as a dimension of completeness. Thus, we conclude that there is need for changes in political, administrative, organizational and social, involving health services, community, family, media and training, and awareness among health professional. It appears that the treatment of tuberculosis limited to the clinical aspects of the disease and regular drug intake is not sufficient to solve your problem and prevent abandonment. |

It appears that the treatment of tuberculosis limited to the clinical aspects of the disease and regular drug intake is not sufficient to solve your problem and prevent abandonment.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESA tuberculose continua destacando-se em todo o mundo, apesar da ciência ter permitido chegar ao tratamento adequado e a cura, com o apoio da expansão da estratégia do Tratamento Diretamente Observado de Curta Duração (DOTS) e descentralização das ações de controle para a Estratégia Saúde da Família, a doença tem o abandono e a não adesão ao tratamento como importantes motivos de insucesso terapêutico, o que ocasiona implicações para saúde pública e para a saúde do próprio indivíduo, pelas suas diversas repercussões. Considerando a maneira como a Equipe de Saúde da Família integra as ações para o controle da tuberculose bem como no cuidado prestado ao paciente e sua família, esta pesquisa objetivou analisar, no âmbito da Atenção Primária em Saúde, os fatores relacionados ao abandono do tratamento da tuberculose na visão do doente, no município de João Pessoa-PB, segundo a dimensão do enfoque familiar. Trata-se de uma pesquisa avaliativa de natureza qualitativa. A construção do material empírico se deu através de entrevistas gravadas, no período outubro e novembro de 2009, utilizando-se a História Oral Temática como referencial metodológico. Foram entrevistados sete usuários, que tiveram o abandono como critério de encerramento para o tratamento da TB. Após análise do material foi produzida uma grande categoria: Fatores relacionados à descontinuidade do tratamento de Tuberculose segundo a percepção do doente de TB, da qual emergiram duas categorias empíricas: Vulnerabilidade social e o abandono do tratamento da tuberculose; O cuidado integral da Equipe de Saúde da Família para com o doente de tuberculose: fortalezas e fragilidades. A trajetória da pesquisa revelou que o vínculo com a Equipe de Saúde da Família é fundamental para o êxito do tratamento podendo comprometer a continuidade do mesmo. Identificou-se a necessidade da equipe de saúde conhecer o projeto de felicidade do indivíduo e de sua família, para melhor conhecer e compreender comportamentos e valores relacionados ao processo saúde-doença. Também se observou que as consultas são importantes estratégias de monitorização do doente e de construção de vínculo. Constatou-se que é imperativo que se envolva o paciente e sua família no processo do cuidado, trazendo à tona a necessidade da prática da co-gestão e do cuidado integral e humanizado, além de incentivar parcerias interdisciplinares e de ações intersetorias. A análise das condições de vida dos colaboradores demonstrou que todos se encontravam em situação de vulnerabilidade social, apontando que a TB está ligada a baixos níveis socioeconômicos, o que leva à reflexão que o doente necessita de cuidado percebido como uma dimensão da integralidade. Desse modo, conclui-se que há necessidade de mudanças de natureza política, administrativa, organizacional e social, envolvendo os serviços de saúde, comunidade, familiares, meios de comunicação e formação, além da sensibilização dos profissionais de saúde.
