Representações sociais acerca do uso do cigarro convencional e do cigarro eletrônico: uma análise comparativa

Resumo: Similar to conventional cigarettes, electronic cigarettes are a device that releases nicotine and has been used as an auxiliary tool for smoking cessation, although scientific data on its efficacy are scarce. In this dissertation, literature review and empirical studies were conducted. In the first study, whose objective was to conduct a literature review on the use of electronic cigarettes as a tool for smoking reduction or cessation, searches were conducted at the Scopus, Scielo and PubMed databases until June 2020, and included only empirical studies dealing with the use of electronic cigarettes for smoking reduction or cessation . As for the results of the review, a great acceptance of the lay population was identified, which sees electronic cigarettes as harmless and reinforces the idea of a new social identity for vapers (e-cigarette users); while health professionals highlight the importance of information based on scientific evidence consistent with the device; to the extent that studies on the efficacy of electronic cigarettes and their long-term safety remain inconclusive. For empirical research, studies were conducted with mixed method, collection and analysis of quantitative-qualitative data. In the second study, conducted to access and compare the social representations of electronic and conventional cigarettes, which included 119 participants from the general population, a sociodemographic questionnaire and the technique of free association of words (with the stimuli "Electronic Cigarette" and "Conventional Cigarette" were used as instruments. In the results of the study, it was noticed that the representations about electronic cigarettes are mostly related to an idea of modernity and youth, relating it to recreational use; while representations about conventional cigarettes are more consistently related to a negative image, unpleasant factors and even the consequent health harms. In the third study, conducted with the objective of investigating the positioning of participants in the face of electronic cigarettes and performing a screening of their mental health, which had 154 participants from the general population, the instruments used were the sociodemographic questionnaire, open questions about electronic and conventional cigarettes, the Hospital Anxiety and Depression Scale and the Life Satisfaction Scale; the data were analyzed with spss software. Regarding the results of the study, no significant differences were found between the participants who were smokers and nonsmokers; most participants believe that conventional cigarettes are more harmful than electronic cigarettes; 41.2% of the participants who used e-cigarettes did not have their own device; most participants believe that electronic cigarettes can be used for smoking cessation, although this was not the reason for the beginning of use for 49% of the participants; most users of ecigarettes only do not consider themselves "smokers"; most participants believe that the use of electronic and conventional cigarettes is used to calm the nerves and that the use of electronic cigarettes occurs for reasons such as: having an environment is conducive, for group integration, having a technological appearance, odor that does not bother or because they believe they are harmless devices. In addition, the representation of most participants recognized the harms related to electronic cigarettes and their composition different from conventional cigarettes, but is unaware that their commercialization is prohibited in Brazil. In general, it was possible to detect the need for longitudinal studies, studies capable of presenting real evidence, in order to clarify doubts and gaps regarding the impacts of the use of electronic cigarettes on people's health and on the efficacy or not of electronic cigarettes for smoking reduction or cessation. It was possible to make some reflections on the role of electronic cigarette for society and how people have introjected the information accessed and shared about the device. Finally, the need for further studies to support the necessary and appropriate regulatory regulations is emphasized, in addition to updating and improving new strategies for prevention and health promotion aligned with control over issues related to electronic cigarettes.

Nesta dissertação, foram realizados estudos de revisão da literatura e empíricos. No primeiro estudo, cujo objetivo foi de realizar uma revisão da literatura acerca do uso de cigarros eletrônicos como ferramenta para redução ou cessação tabágica, foram realizadas buscas nas bases Scopus, Scielo e PubMed até junho de 2020, e incluídos apenas estudos empíricos que tratassem do uso do cigarro eletrônico para redução ou cessação tabágica. Quanto aos resultados da revisão, foi identificada uma grande aceitação da população leiga, que vê o cigarro eletrônico como inofensivo e reforça a ideia de uma nova identidade social para os vapers (usuários do cigarro eletrônico); enquanto que os profissionais de saúde destacam a importância de informações baseadas em evidências científicas consistentes em relação ao dispositivo; na medida em que os estudos sobre a eficácia dos cigarros eletrônicos e sua segurança a longo prazo permanecem inconclusivos. Para as pesquisas empíricas, foram realizados estudos com método misto, coleta e análise de dados quanti-qualitativas. No segundo estudo, realizado para acessar e comparar as representações sociais dos cigarros eletrônicos e convencionais, que contou com 119 participantes da população geral, foram utilizados como instrumentos um questionário sociodemográfico e a técnica de associação livre de palavras (com os estímulos “Cigarro Eletrônico” e “Cigarro Convencional”), cujos dados foram analisados no Iramuteq. Nos resultados do estudo, percebeu-se que as representações acerca do cigarro eletrônico estão majoritariamente relacionadas a uma ideia de modernidade e juventude, relacionando-o ao uso recreativo; enquanto as representações sobre o cigarro convencional estão mais relacionadas de uma forma mais consistente a uma imagem negativa, fatores desagradáveis e até aos consequentes malefícios à saúde. No terceiro estudo, realizado com o objetivo de investigar o posicionamento dos participantes frente ao cigarro eletrônico e de realizar um rastreio da saúde mental deles, que contou com 154 participantes da população geral, os instrumentos utilizados foram o questionário sociodemográfico, perguntas abertas sobre os cigarros eletrônico e convencional, a Escala Hospitalar de Ansiedade e Depressão e a Escala de Satisfação com a Vida; os dados foram analisados com auxílio do software SPSS. Quanto aos resultados do estudo, não foram encontradas diferenças significativas entre os participantes fumantes e não fumantes; a maioria dos participantes acredita que o cigarro convencional seja mais prejudicial que o eletrônico; 41,2% dos participantes usuários de cigarro eletrônico não possuíam um dispositivo próprio; a maior parte dos participantes acredita que o cigarro eletrônico pode ser usada para cessação tabágica, embora esse não tenha sido o motivo para início do uso para 49% dos participantes; a maior parte dos usuários de apenas cigarro eletrônico não se consideram “fumantes”; a maioria dos participantes acredita que o uso de cigarros eletrônicos e convencionais se dá para acalmar os nervos e que o uso de cigarros eletrônicos ocorre por razões como: ter um ambiente é propício, para integração grupal, possuir aparência tecnológica, odor que não incomoda ou por acreditarem que são dispositivos inofensivos. Além disso, a representação da maior parte dos participantes reconheceu os malefícios relacionados aos cigarros eletrônicos e sua composição diferente dos convencionais, contudo desconhece que sua comercialização é proibida no Brasil. De forma geral, foi possível detectar a necessidade de estudos longitudinais, estudos capazes de apresentar evidências reais, a fim de esclarecer dúvidas e lacunas referentes aos impactos do uso de cigarros eletrônicos para a saúde das pessoas e sobre a eficácia ou não do cigarro eletrônico para redução ou cessação tabágica. Foi possível realizar algumas reflexões sobre o papel do cigarro eletrônico para a sociedade e como as pessoas têm introjetado as informações acessadas e compartilhadas sobre o dispositivo.
