Resumo: Introduction: Chronic pain is an important condition because it reaches great part of the population, presenting itself as a major cause of seeking the health service. It is mainly characterized by their long life and strong association with cognitive and affectivemotivational aspects. These aspects have a strong influence on the perception and experience of pain. The meanings attributed to pain reflect the varieties of beliefs that patients have regarding to it. and consist in the cognitive-affective systems that establish multiple relationships with the perception and behavioral responses related to pain. Objectives: The study aimed to explore, describe and analyze the meanings attributed to chronic pain by a group of patients who attend public and private health services in the city of João Pessoa. Method: Participated in the sample of non-probabilistic and by convenience, 50 patients of public health care and 50 patients from private services,. There were two types of interview: free association and semi-structured interview. The free association had as a stimulus the word "pain", and requested five answers that should be prioritized according to their importance. The semi-structured interviews looked for answers to some guiding questions as: "What does your pain mean to you?" And "What is the meaning your pain has in your life?" We also assessed the pain characteristics by respondents, including the type of pain and the intensity experienced by a numeric pain scale, health problems presented, and at the end, sociodemographic data on age, gender and education. Results: In the resulting analysis of free associations, five main types of meanings were found: body condition; negative emotion, subjective negative consequences, negative functional consequences, and adaptation and coping. These meanings differ in their use in both types of services, public and private, which showed distinct patterns of meaning of pain. Predominated in the private service a semantic pattern focused on pain and emotion, as well as their consequences, with an emphasis on the facts and objective consequences of pain and its implications in the effective manipulation of the physical and social world of the patient. In public offices there was a greater emphasis on adaptation and coping with the pain problem by seeking the solution in the form of treatment or redefinition of the pain. x Analyzing the answers of the semistructured interview, done through the software ALCESTE, the speech of the patients showed two standards. The first focused on their pain, their emotions and its consequences, was composed of a semantic core that relates to emotion, expressed either as a corporal sensation, or as a valuation of this sensation (sadness and suffering), or as a consequence of this feeling (limits and impossibilities). The other observed speech showed the capacity of facing the pain with an adaptive coping and seeking a way to solve it, like the action to go see a doctor, take some medicine, and other procedures in search of improvement. His semantic core refers to the situation and the way to face it. Conclusion: Knowing the meanings attributed by the patient to chronic pain, makes a better professional work possible with the patient, adopting the most appropriate therapies, facilitating patient adherence to treatment, as well as providing an adequate coping strategies adopted by the patient regarding to pain.

É caracterizada principalmente por sua duração prolongada e forte associação com aspectos cognitivos e afetivomotivacionais. Tais aspectos exercem forte influência na percepção e experiência de dor. Os significados atribuídos à dor refletem as diversas crenças dos doentes frente à dor e constituemse em sistemas cognitivo-afetivos que estabelecem múltiplas relações com a percepção e as respostas comportamentais frente à dor. Objetivos: o estudo teve por objetivo explorar, descrever e analisar os significados atribuídos à dor crônica por um grupo de pacientes freqüentadores de serviços de saúde públicos e privados da cidade de João Pessoa. Método: Participaram da amostra não-probabilística e por conveniência, 50 pacientes do serviço público de saúde e 50 pacientes de consultórios particulares. Foram realizados dois tipos de entrevista: associação livre e entrevista semi-estruturada. A associação livre teve como palavra estímulo dor , sendo solicitadas cinco respostas que deveriam ser hierarquizadas em função de sua importância. A entrevista semi-estruturada procurou respostas para algumas perguntas norteadoras como: O que essa dor significa para você? e O que essa dor significa na sua vida? . Foram também avaliadas as características da dor apresentada pelos entrevistados, incluindo o tipo de dor e a intensidade da dor sentida, através de uma escala numérica de dor; os problemas de saúde apresentados; e ao final, dados sociodemográficos de idade, sexo e escolaridade. Resultados: Nas análises resultantes das associações livres, cinco tipos principais de significações foram observadas: afecção do corpo; emoção negativa; conseqüências subjetivas negativas; conseqüências funcionais negativas; e adaptação e enfrentamento. Estes significados diferem em seu uso nos dois tipos de consultório, públicos e privados, que apresentaram padrões distintos de significação da dor. Nos consultórios particulares predominou um padrão semântico focado na dor e na emoção, assim como nas suas conseqüências, com uma ênfase maior nas conseqüências fácticas e objetivas da dor e nas suas implicações efetivas na instrumentalização do mundo físico e social do paciente. Nos consultórios públicos observou-se uma ênfase maior na adaptação e enfrentamento do problema álgico, através da busca de resolução na forma de viii tratamento ou de ressignificação da dor. Nas análises das respostas à entrevista semi-estruturada, feitas através do software ALCESTE, o discurso dos pacientes apresentou-se configurado em dois padrões. Um primeiro, focado sobre a própria dor, as suas emoções e as suas conseqüências, foi composto por um núcleo semântico que remete à emoção, expressa seja como sensação corpórea, seja como valoração dessa sensação (tristeza e sofrimento), seja como consequência dessa sensação (limites e impossibilidades). O outro discurso observado revelou-se voltado para o enfrentamento adaptativo da dor, buscando uma forma de resolvê-la, como a ação de procurar um médico, de tomar remédio, e outros procedimentos em busca de melhora. Seu núcleo semântico, remete assim à situação e ao seu enfrentamento.
