A presença de fragilidade e comorbidades são comuns nos idosos em HD, associados a piores desfechos, porém sua correlação com patência de FAV é ainda desconhecida. Para o idoso, questiona- se se os benefícios da FAV seriam alcançados, com o mínimo de exposição e danos. Avaliar a relação entre a patência da FAV em idosos incidentes em HD e o índice de comorbidade de Charlson, a escala clínica de fragilidade e o diâmetro dos vasos receptores. Coorte retrospectivo de abril de 2019 a junho de 2022, com pacientes portadores de DRC em HD e idade superior a 60 anos, com FAV confeccionada posteriormente ao início da HD e submetidos a um mapeamento vascular através de ultrassonografia Doppler no pré-operatório da FAV. Foram avaliados 89 pacientes com média de idade 70.43 ± 7.74. Entre as FAV, 46,1% foram braquiocefálicas, 33,7% radiocefálicas e 20,2% basílicas. A patência primária ocorreu em 64% das fístulas, 47,6% com estenose e 40,2% sofreram angioplastia. As basílicas apresentam mais intervenções para patência, em relação às radiocefálicas e braquiocefálicas, nesta mesma sequência (22,2% vs. 16,7% e 9,8%; p=0,05) e as braquiocefálicas se mostraram com correlação independente com estenose (OR 0,06; (0,009-0,404). Charlson ≥5 esteve presente em 18,4% dos pacientes e fragilidade em 36%, e não se associaram à patência de FAV. Houve uma associação independente entre estenose e diâmetro de veia (OR 0,381; 0,149-0,973), fragilidade (OR 3,27; (1,01-10,65) e óbito (OR 4,36; (1,26-15,15). O índice de comorbidade de Charlson e a Escala Clínica de Fragilidade não apresentaram correlação com a patência de FAV e os maiores diâmetros de veia apresentaram associação com maior patência. Pacientes mais frágeis apresentaram mais frequência de intervenções em FAV e a presença de fragilidade apresentou associação independente com estenose e mortalidade. Esses dados sugerem que em pacientes idosos em HD seja criteriosa a escolha da FAV como primeiro acesso vascular, em virtude do grande número de intervenções e sua associação independente com mortalidade.Vascular access is essential for patients who require hemodialysis (HD) to maintain life. Among the types of vascular access, the arteriovenous fistula (AVF) is identified as the option that offers the best conditions for this type of treatment, considering the number of necessary interventions and operating time. Frailty and comorbidities are common in older people undergoing HD and associated with worse outcomes, but their correlation with AVF patency is still unknown. For older people, the question arises whether the benefits of AVF would be achieved with minimal exposure and damage. To evaluate the relationship between AVF patency in elderly patients undergoing HD and the Charlson comorbidity index, the clinical frailty scale and the diameter of the recipient's vessels. Retrospective cohort from April 2019 to June 2022, involving patients with CKD undergoing HD over 60 years old, with an AVF created after the start of HD and undergoing vascular mapping using Doppler ultrasound in the pre- surgery operative. 89 patients with a mean age of 70.43 ± 7.74 were evaluated. Among AVFs, 46.1% were brachiocephalic, 33.7% radiocephalic and 20.2% basilic. Primary patency occurred in 64% of fistulas, 47.6% with stenosis and 40.2% underwent angioplasty. The basilicas present more interventions for patency, concerning the radiocephalic and brachiocephalic ones, in this exact sequence (22.2% vs. 16.7% and 9.8%; p=0.05) and the brachiocephalic ones showed an independent correlation with stenosis (OR 0.06; (0.009-0.404).
