Objetivos: Descrever a SM segundo duas definições em adultos da área urbana e rural do estado de Pernambuco (Brasil) no ano de 2006 e analisar as associações com variáveis biológicas, socioeconômicas e ambientais. Métodos: Estudo transversal que utilizou o banco de dados da pesquisa “I Inquérito estadual sobre doenças crônicas e agravos não transmissíveis: prevalência e fatores de risco”, com amostra de 1.331 adultos de 25-59 anos de idade. A análise estatística foi realizada com comparação de prevalência da SM entre os estratos das variáveis explanatórias com teste do Chi-quadrado e Regressão de Poisson para identificar fatores independentemente associados à síndrome. Resultados: O estudo mostrou prevalência elevada da SM pelas definições da IDF (39,0%) e JIS (43,9%) em Pernambuco, sendo maior na região do interior urbano, quando comparado à região metropolitana do Recife e interior rural. As prevalências da SM apresentaram associação positiva com a idade, o Índice de massa corporal (IMC) e renda familiar per capita, e associação negativa com a escolaridade; não houve associação com sexo, sedentarismo e tabagismo. Conclusões: Neste estudo, a prevalência de SM mostrou-se elevada, principalmente segundo a definição do JIS, sendo maior na região no interior urbano, aumentando com a renda familiar per capita, o avançar da idade e o IMC, e diminuindo com a elevação do nível de escolaridade, fatores a serem considerados para a abordagem da síndrome nessa população.Metabolic syndrome (MS) definitions proposed by the International Diabetes Federation (IDF) and the Joint Interim Statement (JIS) enable to consider biological populational variations. Objectives: To describe MS according two definitions in adults from urban and rural areas of Pernambuco state, Brazil, in 2006, and analyse the associations with biological, socioeconomic and environmental variables. Methods: Cross-sectional study that used the database of the research "Chronic non communicable diseases and injuries state inquiry I: prevalence and risk factors”, with a sample with 1.331 adults 25-59 years of age. Statistical analysis compared MS prevalence among the strata of the explanatory variables using the Chi-squared test and Poisson‟s Regression to identify factors independently associated with the syndrome. Results: The study showed high prevalence of MS in Pernambuco according both IDF (39.0%) and JIS (43.9%) definitions, higher in urban interior than in Recife metropolitan region and rural interior. MS prevalence had a positive association with age, BMI (Body mass index) and per capita family income, and a negative association with level of schooling; there was no association with sex, sedentary lifestyle and smoking. Conclusions: In this study, MS prevalence was high, especially according JIS definition and in the urban interior.
