A ocorrencia de refluxo gastroesofágico (RGE) no pós-operatório dessa cirurgia é relacionada a redução do tônus do esfíncter esofageano inferior (EIE) e a presença de fundo gástrico residual (FGR). A fixação do reservatório gástrico remanescente com os ligamentos gastro-esplênico e gastro-cólico (omentopexia) tem surgido como uma opção técnica para evitar ou diminuir o RGE no pós-operatório de GVL. O objetivo deste estudo foi avaliar a presença de sintomas de RGE, alterações no tônus do EIE, e a presença de FGR em obesos submetidos a GVL com omentopexia. Vinte pacientes obesos foram submetidos a GVL com omentopexia, no período de julho de 2016 a julho de 2017 no Hospital Unimed de Teresina/PI. Avaliação clínica, com questionário específico (escore clínico), Endoscopia digestiva alta (EDA) e manometria esofágica (ME) foram realizadas no pré-operatório e com 90 dias de pós operatório (DPO). RX contrastado (seriografia) de esôfago, estomago e duodeno foram realizadas após o 90o DPO. Utilizamos o teste exato de Fischer para avaliar a correlação entre os sintomas de RGE e as alterações no tônus do EIE ou presença de FGR e para avaliar a correlação simultânea com as 2 variáveis, utilizamos o teste ANOVA. Em todas as análises adotamos o nível de significância de 95,0 % (p < 0,05). A pontuação no escore clínico apresentou redução média de 6,7 no pré-operatório para 2,7 no pós-operatório. Na manometria, não houve alterações signicativas no tônus do EIE, com valores médios de 26,04 mmhg e 27,07 mmhg nos 2 tempos. No rx contrastado, o FGR foi identificado em 3 casos. Não houve correlação estatística entre as alterações no tônus do EIE, ou a presença de FGR, com o aumento do escore clínico de RGE (nos casos em que isto ocorreu), mesmo quando as situações foram avaliadas simultaneamente. A GVL com omentopexia melhorou o escore clínico de RGE na maiora dos casos e não determinou alterações signicativas no tônus do EIE. A presença de FGR não determinou piora no escore clínico de RGE.CAPESLaparoscopic vertical gastrectomy (LVG) is one of the surgical procedures indicated in the treatment of obesity. The occurrence of gastroesophageal reflux (GER) in the postoperative period of this surgery is related to the reduction of lower esophageal sphincter (LES) tone and the presence of residual gastric reservoir (RGR). Fixation of the RGR with the gastro-spleenic and gastro-colic ligaments (omentopexy) has emerged as a technical option to avoid or decrease GER in the postoperative period of LVG. The aim of this study was to evaluate the presence of GER symptoms, alterations in the LES tone, and the presence of RGR in obese subjects submitted to LVG with omentopexy. Twenty obese patients were submitted to GVL with omentopexy, from July 2016 to July 2017 at Unimed Hospital of Teresina. Clinical evaluation, with a specific questionnaire [clinical score (CS)], upper gastrointestinal endoscopy (UGE) and esophageal manometry (EM) were performed preoperatively and with 90 postoperative days (POD). Esophageal-gastro-duodenal X-ray seriology were performed after the 90th POD. We used Fischer's exact test to evaluate the correlation between GER symptoms and changes in LES tone or RGR presence, and to evaluate the simultaneous correlation with the 2 variables, we used the ANOVA test. In all the analyzes we adopted the level of significance of 95.0% (p <0.05). The CS presented a mean reduction from 6.7 in the preoperative period to 2.7 in the postoperative period. In the EM, there were no significant changes in the tone of the LES, with mean values of 26.04 mmhg and 27.07 mmhg in the 2 times. In the X-ray with contrast, RGR was identified in 3 cases. There was no statistical correlation between changes in the LES tone, or the presence of RGR, with the increase in the GER clinical score (in cases where this occurred), even when the situations were evaluated simultaneously. GVL with omentopexy improved the clinical GER score in most cases and did not determine significant changes in LES tone.
