Nesta população, a frequência de bacilos Gram-negativos (BGN) produtores de β- lactamase de espectro estendido (ESBL) e carbapenemases vêm aumentando nos últimos anos e vêm sendo considerada uma grave ameaça ao êxito do tratamento antimicrobiano. Assim, o objetivo do estudo foi descrever os fatores relacionados à ICS, frequência de bactérias e fungos isolados, detecção de genes de resistência, mortalidade, e tempo e custo de internamento hospitalar em pacientes assistidos em hospital de referência para tratamento de câncer no Estado de Pernambuco. Dados clínicos e epidemiológicos foram coletados dos pacientes portadores de malignidades, tais como, dados biológicos, sociodemográficos, laboratoriais e taxa de mortalidade e custo hospitalar. Hemoculturas positivas de pacientes com ICS foram coletadas e submetidas à identificação fenotípica dos microrganismos e posteriormente a confirmação através de testes automatizados. As amostras foram estudadas para detecção de genes de resistência a β-lactamases blaSHV, blaTEM, blaCTX-M, blaKPC, blaGES, blaNDM, blaIMP, blaVIM, blaSPM, blaGIM e blaSIM e as oxicilinases blaOXA-48 e blaOXA-58 e polimixina mcr em bacilos Gram-negativos e de genes mecA, vanA e vanB em cocos Gram-positivos. Nos isolados que foram resgatados fungos, testes de identificação e suscetibilidade foram realizados. Hemoculturas positivas de quarenta e cinco pacientes foram recuperadas, no período de 2018 a 2020, sendo a maioria dos indivíduos mulheres, com média de idade de 49 anos e apresentando tumores sólidos. A taxa de mortalidade geral, de mortalidade em 30 dias e de letalidade foram 53,3%, 28,8% e 22,2% respectivamente. O tempo médio de hospitalização foi de 34 dias (1-80 dias), com custo médio hospitalar foi de U$ 21.297,28. De 45 pacientes incluídos na amostra, foram obtidos 55 isolados microbianos recuperados de hemoculturas. Dos isolados obtidos, 38 eram Gram-negativas, 13 Gram-positivas e quatro espécies de leveduras. Escherichia coli e Klebsiella pneumoniae foram os BGN mais frequentes e entre estas espécies, os genes mais encontrados foram blaOXA-48 e blaSIM. Os genes blaGES, blaNDM e blaKPC foram encontrados na maioria dos BGN, já blaTEM e blaCTX foram os genes mais comuns entre os ESBL. Foi documentado a ocorrência de infecções polimicrobianas em cinco casos, incluindo as espécies Pseudomonas aeruginosa, Escherichia coli, Klebsiella pneumoniae, Acinetobacter baumanii, Staphylococcus coagulase negativo (SCoN) e Candida spp. Os genes mecA e vanA foram encontrados em isolados de Staphylococcus spp. e Enterococcus faecium, respectivamente. Espécies de Candida spp. apresentaram alta resistência à voriconazol e fluconazol. Na análise univariada, ICS esteve associada às variáveis: inserção de dispositivos invasivos, comorbidades, quimioterapia, maior tempo de terapia antimicrobiana empírica e, registro de intervenção cirúrgica. Após análise multivariada a condição de apresentar comorbidade permaneceu no modelo, como fator de risco para ICS em pacientes portadores de câncer (p=0,002). Pode-se concluir que indivíduos internados para tratamento de câncer e apresentando comorbidades tiveram maior risco de apresentar ICS causadas por BGN multidroga resistentes (MDR) que por sua vez traz impacto negativo nas taxas de mortalidade e, que medidas de identificação e contenção de disseminação através de precauções de contato são importantes, haja vista que a velocidade de emergência destes patógenos vêm apresentando alta disseminação de resistência a carbapenemases e ESBL identificadas limitando as opções do arsenal terapêutico.CAPESBloodstream infections (BSIs) are one of the most frequent complications in cancer patients. In this population, the frequency of gram-negative bacilli (GNB) producing extended-spectrum β- lactamase (ESBL) and carbapenemases has increased in recent years and has been considered a serious threat to the success of antimicrobial treatment. Thus, the objective of the study was to describe the factors related to BSI, frequency of isolated bacteria and fungi, detection of resistance genes, mortality, and time and cost of hospital stay in patients assisted at a referral hospital for cancer treatment in the State of Pernambuco. Clinical and epidemiological data were collected from patients with malignancies, such as biological, sociodemographic, laboratory data, mortality rate and hospital cost. Positive blood cultures from patients with ICS were collected and submitted to phenotypic identification of microorganisms and later confirmed by automated tests. The samples were studied for the detection of β-lactamases resistance genes blaSHV, blaTEM, blaCTX-M, blaKPC, blaGES, blaNDM, blaIMP, blaVIM, blaSPM, blaGIM and blaSIM and the oxycylinases blaOXA-48 and blaOXA-58 and polymyxin mcr in Gram-negative bacilli and mecA, vanA and vanB genes in Gram-positive cocci. In isolates that were rescued fungi, identification and susceptibility tests were performed. Positive blood cultures from forty- five patients were recovered from 2018 to 2020, with most individuals being women, with a mean age of 49 years and presenting solid tumors. The overall mortality, 30-day mortality and case fatality rates were 53.3%, 28.8% and 22.2% respectively. The mean hospitalization time was 34 days (1-80 days), with a mean hospital cost of US$ 21,297.28. From 45 patients included in the sample, 55 microbial isolates recovered from blood cultures were obtained. Of the isolates obtained, 38 were Gram-negative, 13 were Gram-positive and four were yeast species. Escherichia coli and Klebsiella pneumoniae were the most frequent BGN and among these species, the most frequently found genes were blaOXA-48 and blaSIM. The blaGES, blaNDM and blaKPC genes were found in most BGN, while blaTEM and blaCTX were the most common genes among ESBLs. Polymicrobial infections were documented in five cases, including Pseudomonas aeruginosa, Escherichia coli, Klebsiella pneumoniae, Acinetobacter baumanii, coagulase negative Staphylococcus (CoNS) and Candida spp. The mecA and vanA genes were found in Staphylococcus spp. and Enterococcus faecium, respectively. Candida spp. showed high resistance to voriconazole and fluconazole. In the univariate analysis, ICS was associated with the variables: insertion of invasive devices, comorbidities, chemotherapy, longer duration of empirical antimicrobial therapy, and surgical intervention record. After multivariate analysis, the condition of having comorbidity remained in the model as a risk factor for BSI in cancer patients (p=0.002).
