Influência da ansiedade materna na percepção da qualidade de vida relacionada à saúde bucal em pré-escolares de Pelotas/RS

Resumo: Dental anxiety is associated with impaired oral health status, once it causes people to rely on oral health self-care and avoid dental treatment, and is also related with an impact on oral health-related quality of life. Parental anxiety may as well influence the child s oral condition and this could negatively influence their parents perception on the child s quality of life. The aim of this cross-sectional study was to determine the occurrence of maternal dental anxiety and its relationship with the child s oral health-related quality of life. Six hundred and eight children aged from 2 to 5 years and their mothers, attending health care centers during the Children Vaccination National Day in Pelotas/RS were included in this study. Demographic information, socioeconomic status and the use of dental care services were assessed by means of a questionnaire. Maternal dental anxiety was measured by Corah´s Dental Anxiety Scale and the child s oral health-related quality of life by the Early Childhood Oral Health Impact Scale (ECOHIS). Clinical examination for recording dental caries (dmf-t), dental trauma (O´brien) and occlusal patterns were performed by calibrated dentists. Data analyses were performed by the Poisson regression model using robust variance (RR; 95%CI, p≤0.05). Dental anxiety was present in 40.5% of the mothers. ECOHIS mean score was 3.3 (dp 5.4) and the total score ranged from 0 to 42 (maximum of 52).After multiple regression analysis, maternal dental anxiety was associated with a higher mean in the parental distress domain by ECOHIS (RR=1.60; IC95% 1.20-2.13). Anxiety association with functional domain and total score means was not confirmed after adjustment. A higher mean in the scores of the overall scale was associated with the presence of cavities (RR=2.26; IC95% 1.75-2.93), low maternal schooling (RR=1.32; IC95% 1.02-1.71), irregular use of dental care services by mothers (RR=1.37; IC95% 1.06-1.77) and the fact that the child had never had a dental appointment (RR=0.46; IC95% 0.35-0,61). In conclusion, the maternal perception of the child s oral health-related quality of life is associated with clinical condition, socioeconomic indicators and the use of dental care services. Anxious mothers tend to feel more upset and guilty because of their children s dental problems.

É possível que a ansiedade dos pais também influencie a saúde bucal dos filhos, impactando negativamente a percepção destes sobre a qualidade de vida relacionada à saúde bucal das crianças. O objetivo desse estudo transversal foi avaliar a prevalência de ansiedade odontológica entre as mães e sua influência na percepção sobre a qualidade de vida relacionada à saúde bucal de seus filhos. Foram incluídas 608 crianças de 2 a 5 anos e suas mães, presentes em Unidades de Saúde de Pelotas/RS, durante o dia da Campanha Nacional de Vacinação. As mães responderam a um questionário contendo informações socioeconômicas, demográficas e sobre uso de serviços odontológicos. A ansiedade ao tratamento odontológico foi mensurada pela Dental Anxiety Scale, de Corah, e a qualidade de vida relacionada à saúde bucal das crianças com o Early Childhood Oral Health Impact Scale (ECOHIS).O exame clínico das crianças foi realizado por Cirurgiões-dentistas calibrados quanto à condição da coroa (índice ceo-d da OMS), quanto à presença de traumatismos dentários (O´brien) e de maloclusão. Os dados foram avaliados através do modelo de regressão de Poisson, usando variância robusta (RR:95%IC, p≤ 0,05). A prevalência de ansiedade odontológica entre as mães foi 40,5%. O escore médio do ECOHIS foi 3,3 (DP=± 5,4) e o total variou de 0 a 42 (máximo possível 52). Após análise multivariada, encontrou-se que o fato de a mãe ser ansiosa causou maior média de impactos no domínio estresse dos pais avaliado pelo ECOHIS (RR=1,60; IC95% 1,20-2,13). A associação da ansiedade com maiores médias no domínio funcional e no escore total, encontrada na analise bivariada, não se confirmou na múltipla. Com relação ao escore total do ECOHIS, foi influenciado pela presença de cavidades de cárie (RR=2,26; IC95% 1,75-2,93), baixa escolaridade materna (RR=1,32; IC95% 1,02-1,71), uso não regular de serviço odontológico pela mãe (RR=1,37; IC95% 1,06-1,77) e com o fato de a criança nunca ter ido ao dentista (RR=0,46; IC95% 0,35-0,61). Concluiu-se que a percepção das mães sobre a qualidade de vida relacionada á saúde bucal das crianças é influenciada pela condição clínica, por características socioeconômicas e utilização de serviços odontológicos.
