Defesa : Curitiba, 2008Inclui bibliografiaResumo: A febre reumática (FR) aguda e a sua sequela mais grave, a cardiopatia reumática crônica (CRC) são doenças mediadas por uma resposta imune anormal à infecção de orofaringe pelo Streptococcus pyogenes. A lectina ligante de manana (MBF), colectina dependente de cálcio capaz de ativar o complemento através da via das lectinas, é capaz de ligar-se à N-acetil-glucosamina, molécula presente na parede celular do S. pyogenes e nas válvulas cardíacas humanas. A ativação de complemento pela MBF ocorre através de serina-proteases associadas à MBF (MBF-associated serine proteases - MASPs), principalmente a MASP-2, cujo polimorfismo D120G, presente no éxon 3 do gene MASP2, foi recentemente associado a infecções recorrentes e manifestações auto-imunes. Mutações tanto no éxon 1 quanto na região promotora do gene MBL2 foram associadas a doenças infecciosas e auto-imunes. Em estudos anteriores, observamos uma associação entre altas concentrações de MBF e a CRC. No presente estudo investigou-se a relação do polimorfismo dos genes MBL2 e MASP2 com as principais manifestações clínicas da FR (cardite reumática aguda, cardiopatia reumática crônica e artrite). Um total de 149 pacientes, sendo 43 (28,9%) homens e 106 (71,1%) mulheres, com média de idade de 39,28 + 14,53 anos com história de FR e 147 controles saudáveis, sendo 53 (26%) homens e 94 (64%) mulheres, com média de idade de 38,9 + 13,56 anos foram avaliados para a presença de polimorfismos no éxon 1 (A, B, C, D) e na região promotora X/Y do gene MBL2 através de PCR-SSP e PCR em tempo real. A mutação D120G do gene MASP2 foi determinada através de análise de restrição (PCR-RFFP). Os genótipos relacionados a maior produção de MBF (YA/YA e YA/XA) foram mais freqüentes no grupo total de pacientes com cardite (aguda + crônica), nos pacientes com CRC e nos pacientes com CRA quando comparados com os controles saudáveis (OR 2,44, IC 95% 1,48-4,00, p=0,0006; OR 2,42, IC 95% 1,41-4,16, p=0,001 e OR 2,48, IC 95% 1,09-5,67, p=0,035, respectivamente). Através de análise de regressão logística demonstrou-se que concentrações de MBF>2800 ng/ml aumentam o risco de desenvolvimento de CRC (OR 2,91, IC 95% 1,41-6,03, p=0,003). Ainda, entre os pacientes com história de FR sem sequela cardíaca (n=42), a frequência dos genótipos associados à maior produção de MBF foi mais alta no grupo de pacientes que apresentaram cardite aguda quando comparada aos que não apresentaram esta manifestação clínica (OR: 7,22 IC 95% 1.18-43,99, p=0.031). A artrite foi mais freqüentemente observada nos pacientes portadores de genótipos XAJXA, XA/O e O/O, relacionados à baixa produção de MBF (OR: 0,22, IC 95% 0,05-0,89, p=0,048). A mutação D120G do gene MASP2 foi encontrada em 4/106 (3,77%) pacientes com CRC e em 5/129 (3,88%) controles, todos de maneira heterozigota (p=ns). Nenhum paciente sem a sequela cardíaca apresentou a mutação D120G. Genótipos relacionados com alta produção de MBF parecem estar envolvidos na patogênese da cardite reumática aguda e na sua evolução para a forma crônica da doença. Já a mutação D120G do gene MASP2 parece não apresentar papel relevante na patogênese destas manifestações da FR.Abstract: Rheumatic fever (RF) and its most severe sequela, chronic rheumatic heart disease (CRHD), are mediated by an abnormal immunological host response following a Streptococcus pyogenes oropharyngeal infection. Mannan-binding lectin (MBL), a collectin that activates complement, binds to N-acetylglucosamine, a molecule present on the streptococcus cell wall and on human heart valves. MBL activates complement through its associated serine-proteases MASPs), mainly MASP-2, which D120G polymorphism, present in the third exon of the MASP2 gene, has been recently associated with recurrent infections and autoimmune manifestations. Mutations in the exon 1 and promoter region of MBL2 gene have been associated to several infectious and autoimmune disorders. Previously, we reported an association between high MBL serum concentrations and CRHD. Here we investigate the association between MBL2 polymorphisms and the presence of the major RF clinical manifestations (acute rheumatic carditis, chronic rheumatic heart disease and arthritis). Polymorphisms in exon 1 (A, B, C and D) and in the X/Y promoter region of the MBL2 gene were determined by PCR-SSP and real-time PCR in 149 patients (43/28,9% males, 106/71,1% females, mean age 39,28 + 14,53 years) with a history of RF and 147 controls (53/26% males, 94/64% females, mean age 38,9 + 13,56 years). The presence of the D120G MASP2 mutation was analyzed by PCR-RFLP. Genotypes associated with a higher production of MBL (YA/YA and YA/XA) were more frequent in the patients with carditis (acute + chronic), in the subgroups of patients with CRHD and in the subgroups of patients with history of carditis without valve sequela when compared to controls (OR 2,44, Cl 95% 1,48-4.00, p=0,0006; OR 2,42, Cl 95% 1,414,16, p=0,001 and OR 2,48, Cl 95% 1,09-5,67, p=0,035, respectively). Logistic regression analysis showed that MBL levels>2800 ng/ml increased the risk of CRHD (OR 2,91, Cl 95% 1,41-6,03, p=0,003). Also, among patients without CRHD (n=42/149, 28.2%), the frequency of the high-producing MBL genotypes was higher in the group that developed acute carditis when compared to the patients without this clinical manifestation (OR: 7,22 Cl 95% 1,18-43,98, p=0,031). Arthritis was more frequently observed among carriers of the low-producing MBL genotypes (XA/XA, XA/O and O/O) (OR: 0,22, Cl 95% 0,05-0,89, p=0,048). The MASP2 D120G mutation was detected in 4/106 (3,77%) patients with CRHD and in 5/129 (3,88%) healthy controls, all in heterozygous state (p= ns). None of the patients without cardiac sequela presented the D120G mutation. MBL2 genotypes associated with the high production of MBL seem to be involved in the pathogenesis of rheumatic carditis and its progression to CRHD.
