Defesa: Curitiba, 2008Inclui bibliografiaÁrea de concentraçao: Prática profissional de enfermagemResumo: O objetivo do estudo foi conhecer e descrever o itinerário terapêutico do idoso com câncer, em atendimento ambulatorial hospedados nas Casas de Apoio na cidade de Curitiba – Paraná. Para desenvolver este estudo, a metodologia escolhida foi qualitativa através do método de história oral temática, segundo Meihy (2005). A abordagem interpretativa de Kleinman (1980) ofereceu suporte teórico ao estudo. Segundo o autor, o Sistema de Cuidado à Saúde é formado por três subsistemas interrelacionados e interagindo através da passagem das pessoas por eles: subsistema profissional, popular e folclórico. O campo em que foi desenvolvido o estudo serve como opção de hospedagem aos pacientes e seus familiares no período em que estão em tratamento ambulatorial em Curitiba. Foram realizadas cinco entrevistas e procedidas as etapas de transcrição, textualização e transcriação conforme Meihy (2005). Para os processos analíticos e modo de apresentação das narrativas foram efetuadas as categorizações segundo Minayo (1989). Emergiram 5 categorias: a manifestação da doença/enfermidade; o subsistema profissional no itinerário do tratamento do câncer; convivendo com a doença; subsistema folclórico: presença constante no itinerário de tratamento do câncer; e subsistema popular: seio, esteio e direção. Do conjunto das categorias surgiu o tema: a esperança para o idoso sustenta a garantia do tratamento do câncer no entrelaçar dos subsistemas. Os resultados evidenciaram a passagem dos idosos pelos subsistemas descritos por Kleinman (1980). O itinerário terapêutico do idoso com câncer é cercado de incerteza, dor, espera e tristeza, no entanto a esperança, a família e Deus é a garantia de superação, os amparos necessários. O itinerário percorrido pelo idoso com câncer envolve uma multiplicidade de concepções sobre doença na esfera cultural. Os idosos com câncer, diante do diagnóstico médico, mantêm a determinação no seu conhecimento e nas suas crenças e evidenciam o uso paralelo de práticas adquiridas culturalmente, aliando assim a terapêutica médica à popular de forma complementar e não excludente. Para o subsistema profissional, em especial para a enfermagem, conhecer o itinerário de terapêutico percorrido pelo paciente idoso possibilita o planejamento dos cuidados dentro da realidade sócio-cultural dos pacientes e de suas famílias.Abstract: The study aimed to learn and describe the itinerary of cure of elderly people with cancer in ambulatory service and staying in Casas de Apoio (Support Houses) in the city of Curitiba, state of Paraná. In order to develop this study, the qualitative methodology via the thematic oral history method was chosen, according to Meihy (2005). Kleinman’s (1980) interpretative approach offered technical support to the study. According to the author, the Sistema de Cuidado à Saúde (Health Care System) is formed by three subsystems interrelated and interacting through the subjection of people to them: folkloric, popular and professional subsystems. The field in which the study was developed serves as an option of accommodation for the patients and their relatives during the period in which they are going through ambulatory treatment in Curitiba. Five interviews were carried out, followed by the transcription, contextualization and "transcreation" according to Meihy (2005). For the analytical processes and narrative presentation mode, the categorizations were made according to Minayo (1989). Five categories emerged: the manifestation of the disease/illness; the professional subsystem in the cancer’s course of cure; living together with the disease; folkloric subsystem: constantly present in the cancer’s itinerary of cure; and popular subsystem: core, support and direction. From the group of categories raised the theme: hope, for the elderly, supports the warranty of cure when subsystems are interlaced. The results demonstrate the subjection of the elderly to the subsystems described by Kleinman (1980). The itinerary of cure of the elderly with cancer is surrounded by uncertainty, pain, wait and sadness. However, hope, the family and God are a warranty of cure, the support they need. The itinerary traveled by the elderly with cancer involves a multiplicity of conceptions about the disease in the cultural scope. The elderly with cancer, in face of the medical diagnosis, keep the determination in their knowledge and their beliefs and evidence the parallel use of practices acquired culturally, therefore allying medical to popular therapeutics in a complementary rather than excluding way.
