Defesa: Curitiba, 28/03/2011Bibliografia: fls. 98-107Resumo: As principais dificuldades para o sucesso do transplante renal são as variações sobre a aceitação ao rim transplantado e sua taxa de sobrevivência. Um mecanismo amplamente estudado é o papel dos genes do sistema imune durante este processo, por exemplo o envolvimento das células NK e seus receptores KIR (do inglês Killer-cell Immunoglobulin-like Receptor) devido à sua importância na modulação da resposta destas células contra células alvo. Evidências experimentais demonstraram a infiltração de células NK em rins transplantados e sua atividade aumentada após o transplante.A citotoxicidade das células NK é mediada pela interacção entre os receptores de superfície (por exemplo, KIR) e moléculas de HLA de classe I nas células alvo (por exemplo, HLA-C). De acordo com a hipótese "Missing-Self", a ausência de ligantes específicos de HLA de classe I, na célula alvo, para KIR inibidores poderia levar a aloreatividade das células NK do paciente contra as células do órgão transplantado.Esta ausência do ligante específico é chamada de, em vários estudos, incompatibilidade KIRligante. Foram analisados a presença de 16 genes KIR (através de PCR-SSOP metodologia) e a genotipagem do gene HLA-C (por seqüenciamento e PCR-SSP) em 110 pacientes transplantados renais (57 com a rejeição aguda (RA)) e seus doadores. Além disso, analisamos a influência de três fatores na ocorrência de episódios de rejeição aguda: 1) a identidade KIR (quando o locus está presente em ambos, paciente e seu doador) para 5 loci KIR (KIR2DL1, KIR2DL2 e KIR2DL3, KIR inibidores; KIR2DS1 e KIR2DS2, KIR ativadores), 2) a incompatibilidade KIR-ligante e 3) a expressão de KIR2DS2 e KIR2DS4 em 19 pacientes e 14 doadores (por qRT-PCR). A comparação locus a locus-mostrou, para três genes KIR, que a presença destes foi estatisticamente diferente entre os pacientes com AR e aqueles sem (KIR2DL1, p = 0,01; OR = 4,01; IC 95% = 1,44- 11,17; KIR2DS4, p = 0,02; OR = 0,10; IC 95% = 0,01 - 0,89 e KIR2DS5, p = 0,02; OR = 3,03; IC 95% = 1 ,15-8, 41). Entretanto, quando divididos de acordo com as mismatches HLA (MM) em três grupos (0-2MM, 3MM e 4-6MM) essas diferenças não foram observadas, observou-se apenas uma tendência à significância para o gene KIR2DL1 (0-2MM p = 0,09; e 3 MM p = 0,08). A análise da expressão de KIR2DS2 e KIR2DS4 não mostrou diferenças significativas entre amostras anteriores e pós-transplante (durante imunossupressão, episódio de rejeição aguda, e 30 dias após o transplante). A análise de freqüências gênicas, identidades dos genes, o perfil ativator e inibidor, e além da análise da expressão, sugerem que a citotoxicidade do Natural Killers, durante a rejeição aguda no transplante renal, pode envolver todos os genes KIR e sua interação com todos os seus ligantes, mesmo outras moléculas HLA (por exemplo: HLA-A e HLA-B), e também outros receptores NK.Abstract: The main difficulties to renal transplantation are variations on the transplanted kidney acceptance and its survival rate. One of the mechanisms widely studied is the role of immune system genes during this process, for example KIR genes (Killer-cell Immunoglobulin-like Receptors) because of its importance in the modulation on the response of Natural Killer cells against target cells. Experimental evidence has demonstrated the NK cells infiltration in transplanted organs and its increased activity after the transplantation. The NK cells cytotoxicity is mediated by the interaction between surface receptors (e.g. KIR) and HLA class I molecules on target cells (e.g. HLA-C). According to the hypothesis called "Missing-Self", the lack of specific HLA class I ligands, on target cell, to inhibitory KIR could lead to NK cells alloreactivity from patient against cells from the transplanted organ. This specific ligand lack is called, in several studies, KIR-ligand incompatibility. We analyzed the presence of 16 KIR genes (through PCR-SSOP methodology) and HLA-C genotyping (by sequencing and PCR-SSP) in 110 kidney transplanted patients (57 with acute rejection (AR)) and its donors. Also, we analyzed the influence of three factors on the occurrence of acute rejection episodes: 1) the KIR identity (when the locus is present in both patient and his donor) for 5 KIR loci (KIR2DL1, KIR2DL2 and KIR2DL3, inhibitory KIRs; KIR2DS1 and KIR2DS2, activatory KIRs), 2) the KIR-ligand incompatibility and 3) the expression of KIR2DS2 and KIR2DS4 in 19 patients and 14 donors (by qRT-PCR). The locus-bylocus comparison showed, for three KIR genes, that their presence were statistically different between patients with AR and those without it (KIR2DL1, p = 0,01; OR = 4,01; IC 95% = 1,44-11,17; KIR2DS4, p = 0,02; OR = 0,10; IC 95% = 0,01 – 0,89 and KIR2DS5, p = 0,02; OR = 3,03; IC 95% = 1,15-8,41). Although, when we divided according to HLA mismatches (MM) in three groups (0-2MM, 3MM and 4-6MM) those differences were not observed; we only observed a tendency in KIR2DL1 gene (0-2MM p = 0,09; and 3 MM p = 0,08). The expression analysis for KIR2DS2 and KIR2DS4 has not showed significant differences etween before and after the transplantation (during immunossupression, acute rejection episode and 30 days after transplantation). The analysis of gene frequencies, gene identities, activatory and inhibitory profile, and expression, suggest that the cytotoxicity of Natural Killers, during the acute rejection in kidney transplantation, can involve the KIR genes and its interaction with all their ligands, even others HLA (for example: HLA-A and HLA-B), and also others NK receptors.122f. : il.
