Entre as muitas manifestações clínicas descritas da DC estão as hepatites associadas a DC: a "hepatite celíaca" e as hepatopatias autoimunes. A mais comum é a "hepatite celíaca", caracterizada por aumento nos níveis séricos das transaminases e esta forma de hepatite responde a dieta isenta de glúten (DIG). Já as hepatopatias autoimunes na criança, que incluem a hepatite autoimune, colangite esclerosante primária e a sobreposição de HAI e CEP, não respondem a dieta isenta de glúten, e se não diagnosticadas e não tratadas tem alto índice de morbidade e mortalidade. O objetivo deste estudo foi avaliar a prevalência de DC em crianças e adolescentes com hepatopatias autoimunes, e hepatopatias autoimunes em crianças e adolescentes com DC; também avaliar a presença de alteração bioquímica hepática em pacientes com diagnóstico de DC, bem como sua etiologia e evolução. Foi realizado estudo observacional, analítico, transversal e ambispectivo envolvendo pacientes com idade entre zero a 21 anos, de ambos os sexos e com diagnóstico de DC, HAI, CEP ou síndrome de sobreposiçãoHAI e CEP acompanhados no ambulatório de Gastroenterologia e Hepatologia Pediátrica do Hospital de Clínicas da Universidade Federal do Paraná. Os grupos de estudos foram compostos por 70 pacientes celíacos e 32 pacientes com hepatopatias autoimunes. Nos pacientes com DC a bioquímica hepática foi avaliada ao diagnóstico em 26 pacientes; destes, 5 (19%) pacientes apresentavam alterações na bioquímica hepática, que normalizou após a DIG. No acompanhamento dos pacientes com DC, um paciente evoluiu com CEP. Outro paciente apresentou "hepatite celíaca" após 12 anos de acompanhamento por não aderência à DIG. No grupo com hepatopatias autoimunes um (3%) paciente com diagnóstico de HAI evoluiu com dor abdominal e diarréia crônica e teve o diagnóstico de DC. Os resultados encontrados foram semelhantes aos relatados na literatura. A DC é fator de risco para hepatopatia autoimune e hepatopatia autoimune é fator de risco para DC. Durante o acompanhamento dos doentes celíacos deve-se estar atento para a possibilidade de desenvolvimento de hepatopatia autoimune e vice-versa. Palavras-chaves: Doença Celíaca. Among the celiac diasease (CD) clinical presentations that have been described, are the hepatitis-associated to CD: the "celiac hepatitis" and the autoimmune diseases of the liver. The most common is the "celiac hepatitis", which is characterized by high serum levels of transaminases. This form of hepatitis improves with the adherence to a gluten free diet (GFD). However, the autoimmune diseases of the liver during childhood, which includes autoimmune hepatitis (AIH), primary sclerosing cholangitis (PSC) and overlap syndrome do not respond to a gluten free diet and if not diagnosed and not treated have high morbidity and mortality index. Thus, the aim of this study was to evaluate the prevalence of CD in children and adolescents with autoimmune liver diseases, and the prevalence of autoimmune liver diseases in children and adolescents with CD, as well as to evaluate the presence of liver biochemical alterations in patients diagnosed with CD, as well as its etiology and follow-up. It was a retrospective transversal ambispect study, with patients of both sex aged between zero and 21 years, and diagnosed with CD, AIH, PSC or Overlap syndrome followed at the Pediatric Gastroenterology and Hepatology outpatient clinics of Universidade Federal do Paraná. The group comprised of 70 celiac patients and the 32 chronic autoimmune liver diseases; 26 patients from the CD group had the hepatic biochemistry evaluated to diagnost among them, 5 (19%) had elevated hepatic biochemistry that normalized after GFD. During follow up of the CD patients, 1 patient developed PSC. Another patient presented with "celiac hepatitis" after 12 years of follow up for not coping with the GFD. From the group with autoimmune liver disease, 1 (3%) patient diagnosed with AIH developed abdominal pain and chronic diarrhea during follow-up and was diagnosed with CD. These results are similar to the peer reviewed literature. CD is a risk factor for autoimmune liver disease and autoimmune hepatitis is a risk factor for CD. During the follow up of CD patients, we must be aware that they may develop chronic autoimmune liver disease and vice-versa. Key words: Celiac Disease.Chronic Autoimmune Liver Disease.
