Calciuric levels in pregnant women with high blood pressure

Resumo: OBJECTIVE: Arterial hypertension is one of the main causes of maternal and perinatalmorbimortality in Southern Brazil. Most studies concerning urinary calcium excretionduring pregnancy have found lower levels of calcium excretion in patients withpreeclampsia compared with normotensive pregnant patients and with pregnant patientswith preexisting chronic hypertension and without preeclampsia. The objective of thisstudy was to evaluate urinary calcium excretion in women with hypertensive disordersduring pregnancy.MATERIALS AND METHODS: All pregnant women who were hospitalized at the ObstetricCenter of the Hospital de Clínicas Teaching Hospital for increased blood pressure andbetween December 1998 and September 1999, were submitted to a protocol for collectionof demographic data, blood pressure data, and laboratory exams data (proteinuria;calciuria; and creatinuria from 24-h urine samples; proteinuria/creatinuria ratio; andserum uric acid). We determined hypocalciuria for urinary calcium excretion < 100 mg/24h and significant proteinuria for protein excretion > 300 mg/24h.RESULTS: The 71 patients studied presented averages of 26 years of age, 34 weeks ofgestation, and mean blood pressure of 151/98 mmHg. As to clinical development,35.2% of patients were diagnosed with preeclampsia, 8.5% with chronic hypertension,2.8% with eclampsia, 15.5% with preeclampsia over essential hypertension, 2.8% withHELLP syndrome, and 35.2% with transient hypertension. Patients with preeclampsiashowed 24-h calciuria significantly lower than that of patients with transient increase inblood pressure. There was no significant difference in 24-h calciuria from pregnantwomen with chronic hypertension and from women with preeclampsia.RESULTS: These results suggest that simple mensuration of urinary calcium excretioncan help diagnose hypertensive disorders during pregnancy.

A maioria dos estudos que avaliaram a excreção urinária de cálcio durante a gestação detectaram uma excreção significativamente menor deste íon nas pacientes com pré-eclâmpsia, quando comparadas com gestantes normotensas ou com hipertensão arterial crônica sem préeclâmpsia sobreposta. O objetivo principal deste estudo foi avaliar a excreção urinária de cálcio em pacientes com distúrbios hipertensivos da gestação. MATERIAIS E MÉTODOS: Todas as gestantes que internaram no Centro Obstétrico do Hospital de Clínicas por aumento de pressão arterial, no período de dezembro de1998 a setembro de 1999, foram submetidas a um protocolo para coleta de dados demográficos, pressóricos e de exames laboratoriais (proteinúria, calciúria e creatinúria de 24 horas, relação proteinúria/calciúria e ácido úrico sérico). Estabeleceu-se como hipocalciúria a excreção urinária de cálcio < 100 mg/24h e como proteinúria alterada aexcreção > 300 mg/24h. RESULTADOS: As 71 pacientes estudadas tinham, em média, 26 anos, idade gestacional de 34 semanas e pressão arterial sistólica/diastólica de 151/98 mmHg. Quanto à evolução clínica, 35,2% apresentaram pré-eclâmpsia, 8,5% hipertensãocrônica, 2,8% eclâmpsia, 15,5% pré-eclâmpsia sobreposta, 2,8% síndrome de HELLP e 35,2% evoluiram com normalização da pressão arterial. As pacientes com préeclâmpsia ou eclâmpsia apresentaram uma calciúria/24 horas média (132,8 mg)significativamente menor que as pacientes com aumento transitório de pressão arterial (199,3 mg).
