Resumo: The anatomical and functional study of the fetal heart through ultrasonography allows for the prenatal screening and diagnosis of congenital heart diseases and directs postnatal cardiac follow-up of the newborns, The study of the fetal heart was standardized and universalized in pregnancies under prenatal care at the School Matemity of the Federal University of Rio de Janeiro, through a protocol presented in q this study, which justifies the need to evaluate the performance of the application of nú this protocol in the population studied, which is the general objective of this study. The specific objectives are (1) to describe the follow-up of screening tests for congenital heart defecis and (2) to estimate the accuracy of the fetal heart morphological screening test for the detection of fetal heart abnormalities. It is a prospective, observational, and analytical cohort performed between July 2014 and a August 2015. The eligible population of the study was composed of the patients A. under prenatal care at the institution, who began the same until the second trimester Lá of gestation. We included single fetus pregnancies that performed morphological obstetric ultrasonography with an extended protocol of the fetal heart planes and excluded cases of fetal death and neonatal death before 24 hours of life. Data collection consisted of verifying the medical records regarding the characteristics of the pregnant women, descriptions of the fetal heart morphological screening exams and the outcomes of their newborns. Flowchart was presented with the quantitative of the screening tests performed and the follow up of the cases from the findings, until the interest endpoint of the study, which is the presence or not of congenital heart disease. For this, we consider, among the cases included in the sample, the performance of fetal echocardiography, of the postnatal screening, which consists of the "heart" test, and / or neonatal echocardiography. The results were classified as altered or normal according to the occurrence or not of the heart disease. To determine the accuracy of the screening test, the sensitivity and specificity of the screening test were verified in the study population. A total of 518 fetal heart morphological screenings were performed. There were 2 fetal deaths, 1 neonatal and 58 follow-up losses, which resulted in a final sample of 457 cases, of which 444 (97.15%) had complete follow-up. There were five cases of congenital heart disease, with two true positive case and three false negative cases. From these values we calculated the sensitivity of our screening test of 40% and specificity of 77.6% and overall accuracy of 77.2%. The sample had few cases of heart diseases and only one . was classified as complex, due to the small sample size and its randomness. Probably the extension of the period studied will increase the number of cases and the evaluation of the performance of the method will be magnified. We conclude that (1) the follow-up for screening / diagnosis of congenital heart disease was complete in 97.15% of the final sample and (2) a observed sensitivity was 40%, specificity 77% and overall accuracy 77%.

O estudo do coração do feto foi padronizado e Juniversalizado nas gestações em acompanhamento pré-natal na Maternidade Escola da Universidade Federal do Rio de Janeiro, através de protocolo apresentado neste estudo, o que justifica a necessidade de avaliação do desempenho da aplicação deste protocolo na população estudada, que é o objetivo geral deste estudo. Os objetivos específicos são (1) descrever o seguimento dos exames de rastreio das cardiopatias congênitas e (2) estimar a acurácia do teste de rastreio morfológico do coração fetal na detecção de anomalias cardíacas fetais. Trata-se de coorte prospectiva, observacional, e analítica realizada entre julho de 2014 a agosto de 2015. A população elegível do estudo foi composta pelas pacientes em acompanhamento pré-natal na instituição, que iniciaram o mesmo até o segundo trimestre de gestação. Foram incluídas as gestações com feto único, que realizaram ultrassonografia obstétrica morfológica com protocolo estendido dos planos do coração fetal e excluídos os casos de óbito fetal e óbito neonatal antes de 24 horas de vida. A coleta dos dados consistiu no registro das informações presentes nos prontuários quanto às características das gestantes, descrições dos laudos dos exames de rastreio morfológico do coração fetal e os desfechos dos seus recém-nascidos. Foi apresentado fluxograma com o quantitativo dos exames de rastreio realizados e o seguimento dos casos a partir dos achados encontrados, até o desfecho de interesse do estudo, que é a presença ou não de cardiopatia congênita. Para isso consideramos dentre os casos incluídos na amostra a realização da ecocardiografia fetal, do rastreio pós-natal, que consiste no teste do “coraçãozinho”, e/ou da ecocardiografia neonatal. Os resultados foram classificados em alterados ou normais conforme a ocorrência ou não das cardiopatias. Para determinar a acurácia do teste de rastreio foi verificado na população estudada a sensibilidade e especificidade do teste de rastreio. Foram realizados 518 exames de rastreio morfológico do coração fetal. Ocorreram 2 óbitos fetais, 1 neonatal e 58 perdas de seguimento, o que resultou numa amostra final de 457 casos, dos quais 444 (97,15%) casos tiveram o seguimento completo. Houve cinco casos de cardiopatia congênita, com dois casos verdadeiro positivos e três casos falsos negativos. A partir destes valores calculamos a sensibilidade do nosso teste de rastreio de 40% e especificidade de 77,6% e acurácia global de 77,2%. A amostra apresentou poucos casos de cardiopatia e dentre estes somente um foi classificado como complexo, devido ao reduzido tamanho amostral e sua aleatoriedade. Provavelmente a extensão do período estudado aumentará o número de casos e a avaliação do desempenho do método será magnificada.
