NURSING ACTIONS IN THE PREVENTION OF INFECTIONS ASSOCIATED WITH THE USE OF CATHETERS IN INTENSIVE CARE UNIT

Resumo: Introduction: Infections related to the use of catheters in the Intensive Care Unit (ICU) represent a reality of high rates, which are consolidated as a public health problem of which the nursing team characterizes itself as a potential transforming agent from the establishment of specific care measures with these devices. Objective: To clarify the nurses’ actions in the prevention of infections associated with the use of catheters in the ICU. Methodology: Descriptive study of the integrative review type carried out in the years 2015 to 2019, from March to May, in the databases MEDLINE, LILACS and BDENF. Elaborated from 6 stages. The analysis of the studies was done by reading titles, abstracts and complete articles, once they answered the determined objective. Subsequently, the synthesis of the results was presented in tables, followed by the discussion based on thematic categories identified through the Content Analysis Technique. Results: Through the selected studies, it was observed a range of good practices that can be used by nursing in guaranteeing the quality of care with the devices, whether they refer to the technical aspects of handling or not, such as hand hygiene, use of PPE’s, double checking, disinfection strategies, good communication between professionals and methods to work on factors that interfere in the handling of catheters, such as stress, insecurity and exhaustion. Conclusions: It is suggested to intensify the standardization of bundles and measurement protocols, as well as to improve their coverage, so that they consider the various stages of handling the catheters. There is a need to understand a set of factors that interfere with adherence and dissemination of good nursing practices in this context. It is emphasized the need for studies that detail in-depth the implementation of good practices, and discuss factors that affect the success of their implementation. Objetivo: Aclarar las acciones de las enfermeras en la prevención de infecciones asociadas al uso de catéteres en la UCI. Metodología: Estudio descriptivo del tipo de revisión integradora realizado en los años 2015 a 2019, de marzo a mayo, en las bases de datos MEDLINE, LILACS y BDENF. Construido a partir de 6 etapas. El análisis de los estudios se realizó mediante la lectura de títulos, resúmenes y artículos completos, una vez que respondieron al objetivo determinado. Posteriormente, la síntesis de los resultados se presentó en tablas, seguida de la discusión basada en categorías temáticas identificadas a través de la Técnica de Análisis de Contenido. Resultados: A través de los estudios seleccionados, se observó una gama de buenas prácticas que la enfermería puede utilizar para garantizar la calidad de la atención con los dispositivos, ya sea que se refieran a los aspectos técnicos del manejo o no, como la higiene de las manos, el uso de EPP, doble verificación, estrategias de desinfección, buena comunicación entre profesionales y métodos para trabajar en factores que interfieren en el manejo de los catéteres, como el estrés, la inseguridad y el agotamiento. Conclusiones: Se sugiere intensificar la estandarización de los bundles y protocolos de medición, así como mejorar su alcance, para que consideren las diversas etapas de manipulación con los catéteres. Es necesario comprender un conjunto de factores que interfieren con el cumplimiento y la difusión de buenas prácticas de enfermería en este contexto. Se enfatiza la necesidad de estudios que detallen en profundidad la implementación de buenas prácticas y discutan los factores que afectan el éxito de su implementación. Objetivo: Elucidar as ações do enfermeiro na prevenção de infecções associadas ao uso de cateteres na UTI. Metodologia: Estudo descritivo do tipo revisão integrativa realizado nos anos de 2015 a 2019, no período de março a maio nas bases de dados MEDLINE, LILACS e BDENF. Construído a partir de 6 etapas. A análise dos estudos se deu pela leitura de títulos, resumos, e artigos completos, uma vez que estes respondessem o objetivo determinado. Posteriormente a síntese dos resultados encontrados foi apresentada em tabelas, sendo seguida pela discussão a partir de categorias temáticas identificadas através da Técnica de Análise de Conteúdo. Resultados: Através dos estudos selecionados, observou-se uma gama de boas práticas que podem ser utilizadas pela enfermagem na garantia da qualidade do cuidado com os dispositivos, sendo elas referentes aos aspectos técnicos do manuseio ou não, como higiene das mãos, uso de EPI’s, dupla checagem, estratégias de desinfecção, boa comunicação entre profissionais e métodos para trabalhar fatores interferentes no manejo dos cateteres, como estresse, insegurança e esgotamento. Conclusões: Sugere-se a intensificação da padronização de bundles e protocolos de medidas, bem como melhoria na abrangência destes, para que considerem às várias etapas de manejo com os cateteres. Percebe-se a necessidade de entender um conjunto de fatores que interferem na adesão e disseminação das boas práticas de enfermagem nesse contexto. Enfatiza-se a necessidade de estudos que detalhem de modo aprofundado a realização das boas práticas, e discutam fatores que afetam o sucesso da implantação das mesmas.
