Primary health care and violence against elderly people: integrative review

Resumo: Introduction: Population aging is a global reality. It is a natural process during the period of human development, characterized by progressive cell loss and functional decline of the organism, associated with a greater probability of living with chronic diseases and violence. Objective: To verify the scientific production regarding violence against elderly people in Primary Health Care spaces, positioned as the coordinator of the Unified Health System. Methodology: This is an Integrative Review study, whose methodology is based on Evidence-Based Practice, using the databases: Latin American and Caribbean Literature in Health Sciences (LILACS), Medical Literature Analysis and Retrieval Online System (Medline) and Nursing Database (BDENF) and having as inclusion criteria articles published in the last 5 years (2018-2023), in Portuguese, English and Spanish. Results: Eight articles were selected for qualitative synthesis. The selected works indicate a level of evidence between 2, 4 and 5, that is, 62.5% of the findings have level of evidence 4, according to the Oxford Classification Center for Evidence-based Medicine. They were also grouped into thematic axes, namely: Violence and its manifestations, Professional training and its (dis)continuity, Care in Primary Health Care regarding situations of violence against the elderly and its impasses and Invisible caregiver health. Conclusions: It was possible to observe that there are public policies aimed at this problem (health and social), however, public services still timidly implement what is recommended by legislation. We found that there are proposed actions to care for this risk group, however, they need to be more effective and the continuing education strategy in these spaces could be a powerful resource for better care management. Es un proceso natural durante el período del desarrollo humano, caracterizado por la pérdida progresiva de células y deterioro funcional del organismo, asociado a una mayor probabilidad de vivir con enfermedades crónicas y violencia. Objetivo: Verificar la producción científica sobre la violencia contra las personas mayores en los espacios de Atención Primaria a la Salud, posicionada como coordinadora del Sistema Único de Salud. Metodología: Se trata de un estudio de Revisión Integrativa, cuya metodología se basa en la Práctica Basada en la Evidencia, utilizando las bases de datos: Literatura Latinoamericana y del Caribe en Ciencias de la Salud (LILACS), Sistema de Análisis y Recuperación de Literatura Médica en Línea (Medline ) y Base de Datos de Enfermería (BDENF) y teniendo como criterio de inclusión artículos publicados en los últimos 5 años (2018-2023), en portugués, inglés y español. Resultados: Se seleccionaron ocho artículos para la síntesis cualitativa. Los trabajos seleccionados indican un Nivel de Evidencia entre 2, 4 y 5, es decir, el 62,5% de los hallazgos tienen Nivel de Evidencia 4, según la Oxford Centro de Clasificación de Medicina Basada en la Evidencia. También fueron agrupados en ejes temáticos, a saber: Violencia y sus manifestaciones, Formación profesional y su (dis)continuidad, Atención en Atención Primaria a la Salud ante situaciones de violencia contra el anciano y sus impases y Salud del cuidador invisible. Conclusiones: Se pudo observar que existen políticas públicas orientadas a este problema (salud y social), sin embargo, los servicios públicos aún implementan tímidamente lo recomendado por la legislación. Encontramos que existen propuestas de acciones para atender a este grupo de riesgo, sin embargo, necesitan ser más efectivas y la estrategia de educación continua en estos espacios podría ser un recurso poderoso para una mejor gestión de la atención. Conclusões: Foi possível observar que há políticas públicas destinadas a essa problemática (de saúde e social), entretanto, os serviços públicos ainda executam timidamente o que está preconizado pela legislação. Verificamos que existem ações propostas para o cuidado desse grupo de risco, no entanto, há que ser mais efetiva e a estratégia da educação permanente nesses espaços poderá ser um recurso potente para melhor gestão do cuidado.
