Preparation of a multifuncional guide for planning implant-supported rehabilitation: case report

Resumo: | ABSTRACT Introduction: The demand for implant-supported rehabilitations is increasing in order to achieve better aesthetics and masticatory function. Multifunctional guides can assist in guiding bone availability and contribute to planning the three-dimensional insertion of implants, following the principles of reverse planning. Objective: This study aims to describe the predictable results of implant-supported single crowns using multifunctional guides throughout the planning and treatment phases. Case report: A 44-year-old female patient (A.M.F) attended the Dental Prosthesis clinic of the Department of Dentistry/UFRN due to dissatisfaction with her removable prosthesis and smile aesthetics. The examination revealed the absence of tooth 12, with an interocclusal space of 5mm and a mesio-distal distance of 7mm. A guide was created using acrylic resin, which was filled with a stock tooth in the edentulous space. The tooth was then perforated at the center with a spherical drill n the cingulum region and filled with stick gutta percha. A cone beam computed tomography was performed with the guide in position to allow for implant planning, including inclination and favorable positioning, and to assess bone availability in the region. The images obtained provided a clear plan for implant insertion and identified the need for bone graft in an autogenous block due to the absence of bone availability in the region. Conclusion: The use of multifunctional guides can assist in different phases of treatment and allow for greater predictability of results in implant-supported single prosthetic rehabilitations. La guía multifuncional aparece para orientar la disponibilidad ósea y contribuir a la planificación de la instalación tridimensional de los implantes, siguiendo los principios de la planificación inversa. Objetivo: Describir, a través de un caso clínico, la posibilidad de obtener excelentes resultados predecibles en coronas unitarias implantosoportadas, mediante el uso de guías multifuncionales, en la planificación de principio a fin del tratamiento. Reporte de un caso: Paciente A.M.F, 44 años, sexo femenino, acudió a la Clínica de Prótesis Dental del Departamento de Odontología/UFRN quejándose de insatisfacción con su prótesis removible y armonía de sonrisa. El examen clínico y radiográfico reveló la ausencia del elemento 12, con un espacio interoclusal de 5 mm y una distancia mesiodistal de 7 mm. La guía fue realizada en resina acrílica, la cual fue obturada en el espacio edéntulo con un diente estándar. Luego, el diente de esta guía se perforó en su centro con una broca esférica nº 6 (Jet carbide burs) para pieza recta en la región del cíngulo y con ello se rellenó con barra de gutapercha (Maillefer - Dentsply). Posteriormente, el paciente fue derivado para realizar una tomografía computarizada de haz cónico con la guía multifuncional en posición. Las imágenes obtenidas permitieron la planificación para la instalación de los implantes, como la inclinación y el posicionamiento favorable, así como la ausencia de disponibilidad ósea en la región. Conclusion: Las guías ayudan en las diferentes fases del tratamiento y permiten una mayor previsibilidad de los resultados en las rehabilitaciones protésicas unitarias implantosoportadas. Estética Dental.Introdução: Em busca da estética e da função mastigatória, é cada vez mais crescente a procura por reabilitações implantossuportadas. O guia multifuncional surge para orientar a disponibilidade óssea e contribuir no planejamento da instalação tridimensional dos implantes, seguindo os princípios do planejamento reverso. Objetivo: Descrever, por meio de um caso clínico, a possibilidade de obtenção de resultados de excelente previsibilidade em coroas unitárias implantossuportadas, por meio do uso de guias multifuncionais, no planejamento do início ao fim de tratamento. Relato de caso: Paciente A.M.F, 44 anos, sexo feminino, compareceu à clínica de Prótese dentária do Departamento de Odontologia/UFRN com queixa de insatisfação da sua prótese removível e harmonia do sorriso. Ao exame clínico e radiográfico, observou-se ausência do elemento 12, apresentando um espaço interoclusal de 5mm e distância mésio-distal 7mm. Foi confeccionado o guia em resina acrílica, o qual foi preenchido o espaço desdentado com um dente de estoque. Em seguida, o dente deste guia teve seu centro perfurado com uma broca esférica para peça reta na região de cíngulo e com isso, preenchida com guta percha em bastão. Após isso, o paciente foi encaminhado para realizar uma tomografia computadorizada cone beam com o guia multifuncional em posição. As imagens obtidas permitiram o planejamento para instalação do implante, como inclinação e posicionamento favorável, bem como ausência de disponibilidade óssea na região. Conclusão: Os guias auxiliam em diversas fases do tratamento e permitem maior previsibilidade dos resultados em reabilitações protéticas unitárias implantossuportadas.
