The impact of COVID-19 on the health of institutionalized persons

Resumo: Introduction: The aged population, in increasing numbers in Brazil and worldwide, is one of the groups most vulnerable to infection by the new coronavirus. Such fragility is even more evident in those who live in Long-Term Institution for the Aged (LTIA), considering that, in addition to being a fragile part of the analyzed age group, it presents a higher risk of contamination and, still, of geriatric morbidity and mortality . Objective: To identify the impact of COVID-19 on the quality of life of elderly people living in the LTIA Methodology:This is an integrative literature review study, conducted in the months of November and December 2020, summarizing published studies on the subject. We searched the databases Scielo, Lilacs and Medline, using the descriptors "elderly", "COVID-19" and "Long-Term Institution for the Aged". As inclusion criteria, free articles were considered, made available in full and corresponding to the theme analyzed in the study. 16 papers were found. Following these criteria and excluding technical notes and duplicate articles, we verified 4 articles. Results: LTIA, as collective environments, have residents with high vulnerability to infection by the new coronavirus. In those elderly people affected by dementia, delirium and other neurological diseases, social isolation deepens the severity of COVID-19 infection, thus hampering the activities related to the care and assistance provided by caregivers. Furthermore, the adoption of early laboratory screening is characterized as a fundamental measure to enhance the prevention of cases in LTIA and promote infection control in these centers. There is also a deficiency regarding the construction of a protocol with prophylaxis measures and adequate management in a more targeted way within the LTIA. Conclusions:  It is urgent to look more and more closely to the needs of the elderly population, especially the institutionalized group, in order to promote investment policies in comprehensive health care carried out by a multidisciplinary team.

Tal fragilidad es aún más evidente en quienes viven en Instituciones de Atención de Larga Duración para el Anciano (IALDA), considerando que, además de ser una parte frágil del grupo de edad analizado, presenta un mayor riesgo de contaminación y, aún, de morbilidad y mortalidad geriátrica. Objetivo: Identificar el impacto del COVID-19 en la salud de los ancianos que viven en el IALDA. Metodología: Se trata de un estudio de revisión integradora de la literatura, realizado en los meses de noviembre y diciembre de 2020, que resume los estudios publicados sobre el tema. Se realizaron búsquedas en las bases de datos Scielo, Lilacs y Medline, utilizando los descriptores "anciano", "COVID-19" e "Institución de larga duración para ancianos". Como criterio de inclusión se consideraron artículos libres, disponibles en su totalidad y correspondientes a la temática analizada en el estudio. Se encontraron 16 artículos. Siguiendo estos criterios y excluyendo notas técnicas y artículos duplicados, verificamos 4 artículos. Resultados: El IALDA, como entornos colectivos, tiene residentes con alta vulnerabilidad a la infección por el nuevo coronavirus. En aquellas personas mayores afectadas por demencia, delirio y otras enfermedades neurológicas, el aislamiento social profundiza la gravedad de la infección por COVID-19, dificultando así las actividades relacionadas con el cuidado y la asistencia brindada por los cuidadores. Además, la adopción del cribado precoz de laboratorio se caracteriza por ser una medida fundamental para mejorar la prevención de casos en IALDA y promover el control de infecciones en estos centros. También existe una deficiencia en la construcción de un protocolo con medidas de profilaxis y manejo adecuado de forma más focalizada dentro de la IALDA. Conclusiones: Es urgente mirar cada vez más de cerca las necesidades de la población anciana, especialmente del colectivo institucionalizado, con el fin de impulsar políticas de inversión en salud integral y llevadas a cabo por un equipo multidisciplinario.Introdução: A população idosa, em número crescente no Brasil e no mundo, constitui um dos grupos mais vulneráveis à infecção pelo novo coronavírus. Tal fragilidade encontra-se ainda mais evidente naqueles que residem em Instituições de Longa Permanência para Idosos (ILPI), tendo em vista que, além de compor uma parcela fragilizada da faixa etária analisada, apresenta maior risco de contaminação e, ainda, de morbimortalidade geriátrica. Objetivo: Identificar o impacto da COVID-19 na saúde de pessoas idosas residentes de ILPI. Metodologia: Trata-se de estudo de revisão integrativa da literatura, realizado nos meses de novembro e dezembro de 2020, sintetizando estudos publicados sobre a temática. Buscou-se nas bases de dados Scielo, Lilacs e Medline, por meio dos descritores “idoso”, “COVID-19” e “Instituição de Longa Permanência para Idosos”. Como critérios de inclusão foram considerados artigos gratuitos, disponibilizados integralmente e que correspondem à temática analisada no estudo. Foram encontrados 16 trabalhos. Seguindo tais critérios e excluindo-se notas técnicas e artigos duplicados, verificamos 4 artigos. Resultados: As ILPI, como ambientes coletivos, possuem residentes com elevada vulnerabilidade à infecção pelo novo coronavírus. Naqueles idosos acometidos por quadros de demência, delirium e outras doenças neurológicas, o isolamento social aprofunda a gravidade da infecção pela COVID-19, dificultando, assim, as atividades relacionadas à atenção e assistência realizada pelos cuidadores. Ademais, caracteriza-se como medida fundamental a adoção do rastreamento laboratorial precoce para potencializar a prevenção de casos nas ILPI e promover o controle da infecção nesses centros. Percebe-se ainda uma deficiência referente à construção de um protocolo com medidas de profilaxia e manejo adequado de modo mais direcionado dentro das ILPI.
