Xenoenxerto versus enxerto autógeno em cirurgia de levantamento de seio maxilar: uma revisão sistemática.

Resumo: | A sufficient bone structure is required for the installation of an implant in the desired location. Losses of height and bone thickness in the posterior maxilla, the maxillary sinus lift surgery is the preferred technique. This procedure involves placing graft between the sinus membrane and the floor of the maxillary sinus, aiming at recovering the amount of bone making it possible to implant placement. The grafts can be autogenous origin, heterogenous or xenogenous. The autogenous graft is known as the gold standard, but it can present some disadvantages such as limited amounts of bone in some cases, high cost and morbidity areas of the patient. Therefore, further studies are being developed in order to find substitutes for autogenous. One such alternative is the new xenogeneic grafts in which the BioOss® is a type of xenograft bovine origin. The objective was to compare, through a systematic review, the autograft and xenograft (BioOss®), identifying the main advantages and disadvantages, evaluating success rate, resorption capacity, time to implant installation and the rate of survival of implants placed in both the autograft and in xenograft, and / or a combination of both. Search strategies using words and descriptors were made and applied in four research bases in the first time, the articles were selected by title, abstract and inclusion and exclusion criteria. In the second phase, the items were scored by two raters after his complete reading. Thus, shaped articles 18 selected for the following study. It is concluded that the three types of grafts studied in this systematic review, and they autograft, the BioOss®, and the combination of both, are effective in maxillary sinus lift surgery. The time required for installing the implant in most cases was 6 months and the resorption rate is greater than the autogenous BioOss® and the combination of the two. Furthermore, the survival rate of the implants installed was fairly high, and in most cases greater than 90%. |

Em perdas de altura e espessura óssea na região posterior de maxila, a cirurgia de elevação de seio maxilar é a técnica de eleição. Este procedimento consiste na colocação de enxerto entre a membrana sinusal e o assoalho do seio maxilar, visando à recuperação da quantidade de osso tornando possível a instalação do implante. Os enxertos podem ter origem autógena, heterógena ou xenógena. O enxerto autógeno é conhecido como padrão ouro, porém, ele pode apresentar algumas desvantagens como quantidades limitadas de osso em alguns casos, custo elevado e áreas de morbidades no paciente. Por isso, novos estudos estão sendo desenvolvidos com o intuito de achar substitutos para o autógeno. Uma dessas novas alternativas é o enxerto xenógeno, na qual o BioOss® é um tipo de xenoenxerto de origem bovina. O objetivo do trabalho foi comparar, através de uma revisão sistemática, o enxerto autógeno e o xenoenxerto (BioOss®), identificando as principais vantagens e desvantagens, avaliando índice de sucesso, a capacidade de reabsorção, tempo para instalação do implante e a taxa de sobrevivência dos implantes instalados tanto no enxerto autógeno quanto no xenoenxerto, e/ou na combinação de ambos. Estratégias de buscas utilizando palavras e descritores foram feitas e aplicadas em quatro bases de pesquisa, no primeiro momento, os artigos foram selecionados por título, resumo e critérios de inclusão e exclusão. No segundo momento, os artigos foram pontuados por dois avaliadores após sua leitura completa. Dessa forma, 18 artigos forma selecionados para o seguinte estudo. Conclui-se que os três tipos de enxertos estudados nessa revisão sistemática, sendo eles o enxerto autógeno, o BioOss®, e a combinação entre ambos, são efetivos em cirurgias de levantamento de seio maxilar. O tempo necessário para a instalação do implante na maioria dos casos foi de 6 meses e a velocidade de reabsorção foi maior no enxerto autógeno do que no BioOss® e na combinação entre os dois.
