Resumo: The aesthetic demands have been increasing considerably over the years in the routine of clinical practice. The excessive gingiva display, commoly described as “gingival smile”, has received more emphasis in the dental literature in the last years, and has been a complaint of many patients, since such a situation may influence self-esteem and social relationship. For this reason, it is essential that the differential diagnosis be developed so that a specific therapy is established to the patient. Thus, the presente work aimsed to discuss the diagnosis of gingival smile and its main forms of treatment. Na integrative review was carried out and 640 articles were found, and only 13 were used in this review. The Scielo, PubMed, LILACS e MEDLINE databases were used. As results were found as etiologies for the gingival smile factors associated with lip length and activity, clinical crown length, dentoalveolar extrusion and vertical maxillary excess. The possibilities of tratments envolve several therapeutic modalities, among them, gingivectomy or gingivoplasty and orthognatic surgery, as well as botulinum toxin, which can also be considered an option. Therefore, the planning of individualized and integrated tretment for each type of gingival smile is essential.

O excesso de gengiva a mostra, comumente descrita como "sorriso gengival", tem recebido maior ênfase na literatura odontológica nos últimos anos, e tem sido queixa de muitos pacientes, já que tal situação pode influenciar na autoestima e no relacionamento social. Por tal razão, é essencial que o diagnóstico correto do sorriso gengival seja desenvolvido para que uma terapia específica seja estabelecida ao paciente. Sendo assim, o presente trabalho discutiu o diagnóstico do sorriso gengival e as suas principais formas de tratamento. Foi realizada uma revisão integrativa da literatura e foram encontrados 640 artigos, sendo utilizados nessa revisão apenas 13. Foram usadas as bases de dados Scielo, PubMed, LILACS e MEDLINE. Como resultados foram encontrados como etiologia para o sorriso gengival fatores associados ao comprimento e à atividade do lábio, ao comprimento da coroa clínica, à extrusão dento-alveolar e ao excesso vertical da maxila. As possibilidades de tratamentos envolvem diversas modalidades terapêuticas, entre elas, a gengivectomia ou gengivoplatia e a cirurgia ortognática, além da toxina botulínica que também pode ser considerada uma opção.
