Clinical Treatment of Pancreatic Abscess in a Feline - Case Report

Resumo: Pancreatic abscesses are cysts filled with purulent content, caused by bacterial infection. In the feline species, this is considered an uncommon condition. This study describes the clinical, laboratory, ultrasonographic aspects, diagnosis, and treatment of a feline presenting pancreatic abscesses. A nine-year-old male mixed-breed cat was examined due to a history of lethargy and reduced appetite. Physical examination revealed abdominal tenderness and mucosal pallor. Abdominal ultrasound identified the presence of cavitated structures filled with hypoechoic content within the pancreatic tissue. Cytology performed through ultrasound-guided fine-needle aspiration (FNA) indicated the presence of degenerated neutrophils and Gram-negative rods. Bacterial growth of Escherichia coli was observed in the sample culture, confirming compatibility with a pancreatic abscess. Therefore, treatment was initiated with antibiotic therapy, pain control, and anti-nausea measures. Clinical, laboratory, and ultrasonographic resolution was observed five days after treatment initiation. It was concluded that conservative treatment in cases of pancreatic abscesses can be an effective alternative.

Na espécie felina, são considerados uma afecção incomum. O presente relato descreveu os aspectos clínicos, laboratoriais, ultrassonográficos, diagnóstico e tratamento de um felino apresentando abscessos pancreáticos. Um felino, macho, sem raça definida, com nove anos de idade foi atendido com histórico de apatia e hiporexia. Ao exame físico notou-se sensibilidade à palpação abdominal e hipocromia de mucosas. Por meio da ultrassonografia abdominal identificou-se estruturas cavitárias preenchidas por conteúdo hipoecogênico no parênquima pancreático. A citologia realizada por punção aspirativa com agulha fina (PAAF) guiada por ultrassom apontou a presença de neutrófilos degenerados e bastonetes Gram negativos. Na cultura da amostra verificou-se crescimento bacteriano de Escherichia coli, evidenciando que tal achado era compatível com abscesso pancreático, sendo então instituído o tratamento com antibioticoterapia, controle de dor e de náusea. Cinco dias após o início do tratamento observou-se uma resolução clínica, laboratorial e ultrassonográfica.
