Tratamento estético para incisivos permanentes afetados pela hipomineralização molar - incisivo: relato de caso clínico

Resumo: INTRODUCTION: Molar-incisor hypomineralization (MIH) corresponds to a qualitative defect of dental enamel of unknown etiology, but with a multifactorial characteristic, since both genetic, systemic and environmental factors may be involved. Clinically, demarcated and asymmetrical opacities with color ranging from white to brown are observed in first permanent molars, which may or may not affect the permanent incisors. Teeth affected by MIH can present aesthetic alterations, loss of tooth structure, caries lesions and dentin hypersensitivity and can have a negative impact on quality of life. OBJECTIVE: To report the case of a child diagnosed with mild MIH and to present aspects of the dental management of this condition. CASE REPORT: Male patient, melanoderm, 10 years old, with an aesthetic complaint in maxillary anterior teeth and without reports of complications or health problems during the pre-, peri- and postnatal periods, as well as in the first three years of life. In the intraoral clinical examination, demarcated opacities were observed on teeth 11 and 21, with a larger defect on tooth 21, which bothered the patient when smiling. In addition, demarcated opacity was also observed in the upper right first molar, tooth 16. The patient was diagnosed with mild MIH. For the proposed treatment, a sealant was applied with glass ionomer cement to the occlusal surface of tooth 16 and a minimally invasive approach was performed to mimic enamel changes in the maxillary permanent central incisors (microabrasion + application of resin infiltrant + application of flow resin bulkfill). FINAL CONSIDERATIONS: It is important that MIH is diagnosed as early as possible, ensuring that an appropriate treatment can be carried out and in an ideal period of time. This enables adequate preventive measures to be taken and a greater amount of tooth tissue to be preserved.

Clinicamente, são observadas opacidades demarcadas e assimétricas com coloração variando de branca a marrom em primeiros molares permanentes, podendo ou não afetar os incisivos permanentes. Dentes afetados pela HMI podem apresentar alterações estéticas, perda de estrutura dentária, lesões de cárie e hipersensibilidade dentinária e podem ter um impacto negativo qualidade de vida. OBJETIVO: Relatar o caso clínico de criança com diagnóstico de HMI leve e apresentar aspectos do manejo odontológico dessa condição. RELATO DE CASO: Paciente do sexo masculino, melanoderma, com 10 anos de idade, com a queixa estética em dentes anteriores superiores e sem relato de intercorrências ou problemas de saúde durante os períodos pré-, peri- e pós-natal, bem como nos três primeiros anos de vida. No exame clínico intrabucal, foram observadas opacidades demarcadas nos dentes 11 e 21, com defeito de maior dimensão no dente 21, que incomodava o paciente ao sorrir. Além disso, também foi observada opacidade demarcada no primeiro molar superior direito, dente 16. O paciente foi diagnosticado com HMI leve. Para o tratamento proposto, foi realizada a aplicação de selante com cimento à base de ionômero de vidro (CIV) na oclusal do dente 16 e a realização de abordagem minimamente invasiva para mimetizar as alterações de esmalte nos incisivos centrais permanentes superiores (microabrasão + aplicação de infiltrante resinoso + aplicação de resina flow bulkfill). CONSIDERAÇÕES FINAIS: É importante que a HMI seja diagnosticada o mais cedo possível, garantindo que um tratamento adequado possa ser realizado e em um período de tempo ideal.
