Abordagem minimamente invasiva de lesão cariosa não cavitada em metade externa de dentina em molar permanente: um relato de caso

Resumo: INTRODUCTION: Caries disease is defined by an imbalance in the health-disease process, which may or may not be associated with the development of cavitated carious lesions. Among the currently widespread therapeutic and preventive options, in a conservative alternative approach, the use of pit and fissure sealants stands out, which, according to the literature, are effective in stopping caries lesions restricted to the external half of dentin. OBJECTIVE: To report a clinical case of a restoration with resin sealant applied to a carious lesion reaching the outer half of the dentin in a permanent molar of a patient treated at the Dental Clinic of the Federal University of Sergipe, Lagarto. METHODOLOGY: This is a descriptive case report study, with analysis of medical records, performance of restorative procedure, evaluation, and clinical and radiographic follow-up of 14 months. CASE REPORT: Patient J.V.S.S, 12 years-old, male, attended the Dentistry clinic for dental treatment. On clinical examination, a non-cavitated carious lesion was observed on the occlusal surface of element 36 and a slight shading indicative of possible damage to the dentin. The radiographic examination revealed a carious lesion restricted to the outer half of the dentin in this dental element. Therefore, the conduct performed was the application of resin sealant and, over 14 months, 5 clinical and radiographic follow-ups of the dental unit were carried out. Efficacy was noted regarding the stoppage of the carious lesion since, after 14 months of follow-up, no increase in the radiolucent area was observed, nor the occurrence of painful symptoms, periapical involvement or other sign indicative of failure. Clinically, the sealing material remained intact in the pits and fissures. CONCLUSION: In view of this, the success of the conservative procedure used in a young permanent tooth is highlighted, and the importance of periodic clinical and radiographic follow-up is emphasized to maintain the success of the technique in the long term

Dentre as opções terapêuticas e preventivas atualmente difundidas, com enfoque em alternativas conservadoras, destaca-se a utilização de selantes de fóssulas e fissuras que, segundo a literatura, se fazem eficazes na paralisação das lesões de cárie restritas à metade externa de dentina. OBJETIVO: Relatar o caso clínico de uma restauração com selante resinoso aplicado em lesão cariosa atingindo metade externa de dentina em um molar permanente de um paciente atendido na Clínica Escola do curso de Odontologia da Universidade Federal de Sergipe, campus Lagarto. METODOLOGIA: Trata-se de um estudo descritivo de relato de caso, com análise de prontuário, realização de procedimento restaurador, avaliação e acompanhamentos clínico e radiográfico de 14 meses. RELATO DE CASO: Paciente J.V.S.S, 12 anos de idade, gênero masculino, compareceu à clínica de Odontologia para acompanhamento odontológico. Ao exame clínico, observou-se lesão cariosa não cavitada na face oclusal do elemento 36 e ligeiro sombreamento indicativo de possível acometimento em dentina. Ao exame radiográfico, pôde-se observar lesão cariosa restrita à metade externa da dentina neste elemento dentário. Diante disso, a conduta executada foi a aplicação de selante resinoso e, ao longo de 14 meses, foram realizados 5 acompanhamentos clínicos e radiográficos da unidade dentária. Notou-se eficácia quanto à paralisação da lesão cariosa uma vez que, após proservação de 14 meses, não foi observado aumento da área radiolúcida, nem a ocorrência de sintomatologia dolorosa, comprometimento periapical ou outro sinal indicativo de insucesso. Clinicamente, o material selador permaneceu íntegro nas cicatrículas e fissuras.
