Perfil laboratorial de pacientes com HIV acompanhados no Laboratório Central de Saúde Pública (LACEN) em Aracaju/Sergipe

Resumo: The existence of the HIV virus as the cause of the Acquired Immunodeficiency Syndrome (AIDS) has been known for some thirty years, and can be transmitted through sexual, vertical (maternal) or blood. In Brazil, from the beginning of the AIDS or AIDS epidemic until June 2015, there were some 798,366 cases of the disease, showing a linear trend in the increase of infection in less developed regions such as the North-Northeast with prevalence among the young and the elderly. The greatest challenge faced by the clinical staff in seropositive patients is the appearance of resistance to antiretroviral therapy (ART), which may lead to worsening of the disease with evolution to the death of these patients. Considering this, this study aims to evaluate the incidence of seropositives for the HIV virus in the state of Sergipe and its adherence to antiretroviral treatment, with an emphasis on resistance to ART, if any. For this, we conducted a descriptive, observational, retrospective study based on the collection of data from 40 medical records, from the System of Control of Laboratory Tests (Siscel) and Genotyping System (Sisgeno), of HIV seropositive patients. Of the total number of records analyzed, twenty-one individuals (55%) were male and nineteen individuals (48%) were female. The mean age of the patients was 37 years. Regarding the CD4 and CD8 T lymphocyte counts, we observed that CD8 T lymphocytes predominate in number in relation to T-CD4 in both sexes. In the evaluation of the genotyping tests, it was verified that the occurrence of antiretroviral resistance was greater in males, and these data are corroborated by the higher mean viral load in males compared to females. Considering this, we can conclude that men are more susceptible to the complicationssuch as resistance, immunosuppression,which are induced by the failure of the ART, being necessary actions with focus in this patient, aiming at the improvement of their quality of life.

En Brasil, desde el inicio de la epidemia de SIDA o SIDA hasta junio de 2015, se registraron unos 798.366 casos de la enfermedad, mostrando una tendencia lineal en el aumento de la infección en regiones menos desarrolladas como el Norte-Nordeste con Prevalencia entre los jóvenes y los ancianos. El mayor reto al que se enfrenta el personal clínico en pacientes seropositivos es la aparición de resistencia a la terapia antirretroviral (ART), que puede conducir a un empeoramiento de la enfermedad con evolución hasta la muerte de estos pacientes. En este sentido, este estudio tiene como objetivo evaluar la incidencia de seropositivos para el virus del VIH en el estado de Sergipe y su adhesión al tratamiento antirretroviral, con énfasis en la resistencia al TAR, en su caso. Para ello, se realizó un estudio descriptivo, observacional y retrospectivo basado en la recolección de datos de 40 expedientes médicos, del Sistema de Control de Pruebas de Laboratorio (Siscel) y Sistema de Genotipificación (Sisgeno), de pacientes seropositivos para el VIH. Del total de registros analizados, veintiún individuos (55%) eran varones y diecinueve individuos (48%) eran mujeres. La edad media de los pacientes fue de 37 años. En cuanto a los recuentos de linfocitos T CD4 y CD8, se observó que los linfocitos T CD8 predominan en número en relación con T-CD4 en ambos sexos. En la evaluación de las pruebas de genotipificación, se verificó que la incidencia de resistencia antirretroviral fue mayor en varones, y estos datos se corroboran por la mayor carga viral media en varones en comparación con las mujeres. Teniendo en cuenta esto, podemos concluir que los hombres son más susceptibles a las complicaciones inducidas por el fracaso del TAR, siendo acciones necesarias con enfoque en este paciente, con el objetivo de mejorar su calidad de vida.Há cerca de trinta anos sabe-se da existência do vírus HIV como causa da Síndrome de Imunodeficiência Adquirida (SIDA), cuja transmissão pode dar-se por via sexual, vertical (maternal), ou sanguínea. No Brasil, desde o início da epidemia de SIDA ou AIDS até o período de junho do ano de 2015, registrou-se cerca de 798.366 casos da doença, mostrando uma tendência linear no aumento da infecção em regiões menos desenvolvidas como no Norte-Nordeste com uma prevalência entre os mais jovens e os idosos, mas segundo a UNAIDS em todo o país a prevalência é entre 15 e 49 anos. O maior desafio enfrentado pelo corpo clínico ao tratar os pacientes soropositivos é o aparecimento das resistência à terapia com os anti-rretrovirais (TARV), que pode levar ao agravamento da doença com evolução ao óbito destes pacientes. Visto isso, este estudo visa avaliar a incidência de soropositivos para o vírus HIV no estado de Sergipe e sua adesão ao tratamento antirretroviral, com ênfase nas resistências as TARV, caso existam. Para isso, foi realizado um estudo com delineamento descritivo, observacional, retrospectivo baseado na coleta de dados a partir de 40 prontuários, oriundos do Sistema de Controle de Exames Laboratoriais (Siscel) e Sistema de Genotipagem (Sisgeno), de soropositivos para o HIV. Do total de prontuários analisados, vinte e um indivíduos (55%) pertenciam ao sexo masculino e dezenove indivíduos (48%) ao sexo feminino. A média de idade dos pacientes foi de 37 anos. Em relação à contagem de linfócitos T CD4+ e CD8+, observamos que os linfócitos T CD8+ predominam em número em relação aos T-CD4+,em ambos os sexos. Na avaliação dos exames de genotipagem, verificou-se que a ocorrência de resistência aos antirretrovirais foi maior no sexo masculino, sendo que estes dados são corroborados pela média maior de carga viral nos homens em relação as mulheres.
