The Family Heath Strategy is essential in the proper health care, especially for people with chronic diseases such as hypertension, and expanded the access to health services for the Brazilian population. This study aimed to evaluate the satisfaction of hypertensive users with the access to primary health care services, focusing on the geographical and socio-organizational dimensions. It was carried with 15 people with hypertension and residents of a river island in Sergipe. Data were collected through semi-structured interviews, divided in two parts: one with the application of a validated instrument adapted for the same purpose mentioned here, and another through open questions to the interviewees. Data were analyzed through measures of central tendency and variance, as well as thematic analysis of the speeches. Regular results were evidenced for geographical and socio-organizational accessibility, pointing out several barriers related mainly to the geographical and economic situations of individuals. Evaluation help us to reflect on the need to expand access to health through public policies that reduce inequality, beyond structural investment and expansion of primary care, considering the socioeconomic and geographical characteristics of the communities to be covered.
