Efetividade do sistema reciproc no tratamento endodôntico de dentes anteriores com lesão periapical: ensaio clínico pragmático randomizado controlado

Resumo: The technique of root canal instrumentation with a single reciprocating instrument was developed as an attempt to overcome the limitations presented by manual techniques and using continuous rotation. Despite the easiness to execute it, few studies evaluated the effectiveness of reciprocating instrumentation in clinical practice. Therefore, this pragmatic randomized clinical trial evaluated the clinical effectiveness obtained by an endodontic treatment using a reciprocating single file and a single cone approach. One hundred twenty anterior teeth with necrotic pulp and apical periodontitis were randomly instrumented with hand files and filed with lateral compaction technique; or with a single file in a reciprocating movement followed by root filling with a single cone. The apical extension, homogeneity and taper parameters of obturation were classified as ideal or altered, and the occurrence or absence of sealer extrusion was recorded. Postoperative pain was assessed until 7 days after the treatment using a visual analogue scale (VAS) and a verbal rating scale (VRS). The prevalence of pain (score different from 0 at VRS) and flare-up was evaluated. VAS was also used to assess the participants` perceptions regarding their comfort; time spent by procedures, their chewing ability, and general satisfaction. The periapical index was used to classify the periapical status at baseline and after 6 and 12-months of follow-up. The short version of OHIP questionnaire was used to investigate the impact on quality of Life. Data were analyzed through t-tests, Mann-Whitney, and Chi-Square analysis, and the odds-ratio for healing was adjusted using a Logistic Regression model (α = 0.05). No significant differences were observed between the endodontic techniques regarding the root filling quality, sealer extrusion, or postoperative pain incidence. A higher incidence of pain was observed in the first 24 h (≈ 43.3%), with progressive reduction within that time (≈ 1.7% after 7 days). Both treatments also yielded a similar intensity of postoperative pain measured by either VAS or VRS. Low incidences (≈ 3.3%) of flare-up were observed for both protocols. In general, high levels of participant perceptions were observed for all questions, with no difference between the treatments. The drop-out rate was 27,5%, and the overall healing rates were 66% and 75% for 6 and 12-month follow-up periods, respectively. The healing rate was only affected by the periapical index measured at baseline. Both treatment protocols improved the quality of life of patients. In conclusion, the reciprocating system evaluated resulted in similar quality of toot canal filling, incidence and intensity of postoperative pain, flare-up incidence, and healing rates of apical periodontitis than manual instrumentation. The endodontic protocol does not alter the patient’s perception of treatment. .

Apesar da facilidade técnica, poucos estudos avaliaram a efetividade da instrumentação reciprocante na prática clínica. Assim, o objetivo desta pesquisa foi avaliar o desempenho clínico da técnica endodôntica com lima única em movimento reciprocante e obturação cone único. Cento e vinte dentes anteriores que apresentavam quadro clínico de necrose pulpar e periodontite apical foram aleatoriamente instrumentados com limas manuais de aço inoxidável e obturados pela técnica da compactação lateral, ou instrumentados com uma única lima reciprocante e obturados pela técnica do cone único. A extensão apical, homogeneidade e conicidade de cada obturação endodôntica foram classificadas em ideal ou alterada, e a ocorrência ou não de extrusão de cimento endodôntico pelo forame foi também avaliada. A sensibilidade pós-operatória foi avaliada durante os 7 dias seguintes a realização do tratamento, por meio de uma escala visual analógica (EVA) e de uma escala verbal. As incidências de dor (escore diferente de 0 na escala verbal) e de abscesso fênix foram calculadas. A EVA também foi utilizada para avaliar a percepção dos participantes em relação ao tempo e conforto do procedimento; sua capacidade de mastigar nos 7 dias seguintes ao procedimento e sua satisfação geral com o tratamento. O índice periapical foi utilizado para classificar as lesões periapicais tanto na avaliação inicial quanto nas consultas de proservação de 6 meses e de 12 meses. A versão reduzida em língua portuguesa do questionário OHIP foi utilizado para avaliar o impacto dos tratamentos na qualidade de vida. Os testes de Mann-Whitney, Qui-quadrado e o teste T foram utilizados para analisar os dados. Também foi criado um modelo de regressão logística para ajustar a chance de sucesso (razão de chances) do tratamento (α = 0.05). As duas técnicas endodônticas não apresentaram diferenças estatisticamente significantes em relação à qualidade da obturação endodôntica, ao extravasamento de cimento endodôntico pelo forame apical e à ocorrência e severidade de dor pós-operatória. A incidência desta foi maior nas primeiras 24 horas após a realização do procedimento (≈ 43.3%), e reduziu gradativamente com o passar do tempo (≈ 1.7% 7 dias após o tratamento). As duas modalidades de tratamento resultaram em níveis de sensibilidade pós-operatória semelhantes, tanto quando avaliada pela EVA como pela escala verbal. Baixa incidência (≈ 3.3%) de abscesso fênix foi observada nos dois protocolos de tratamento. De modo geral, a percepção do paciente em relação ao tratamento foi alta em todos os quesitos avaliados, sem diferença entre os protocolos de tratamento. A perda de seguimento foi de 27,5%, e a taxa geral de cura das lesões foi de 66% na proservação de 6 meses e 75% na proservação de 12 meses. A única variável que afetou a chances de sucesso do tratamento foi a classificação inicial da lesão no índice periapical. As duas técnicas melhoraram significativamente a qualidade de vida dos pacientes. Conclui-se que o protocolo de tratamento com lima única reciprocante e cone único do sistema reciprocante Reciproc apresentou efetividade semelhante ao tratamento com limas manuais de aço inoxidável e obturação pela técnica da compactação lateral da guta-percha, no que concerne a qualidade da obturação endodôntica, a ocorrência e severidade de sensibilidade pós-operatória, a ocorrência de abscesso fênix e as taxas de cura de lesões periapicais.
