Resumo: This work presents the situation of population aging, suggesting that active aging is a way to improve the individual's quality of life. As an objective, a questionnaire was developed, consisting of 04 dimensions, which address socioeconomic aspects, perceived health, basic and instrumental activities of daily living and social activities, totaling 44 questions, with clear options and unique answers. Also, a model was proposed to calculate a score, using the responses from these dimensions. After that, an application for Android device was developed, which collects the user's responses through pre-established indexes, generating the evaluation score, in order to present the user's current health condition. A test of the use of the application was carried out in a total of 10 people, of all ages, who answered the questionnaire and viewed its results on screen. Given the above, it is clear that the application is valid and that some updates are needed regarding the score used and consolidation of the use of the results, so that these are publicized, with the aim of promoting improvements in the health of the elderly.

Como objetivo, foi desenvolvido um questionário, composto por 04 dimensões, que abordam sobre aspectos socioeconômicos, saúde percebida, atividades básicas e instrumentais de vida diária e atividades sociais, perfazendo um total de 44 questões, de opções claras e de respostas únicas. Também, foi proposto um modelo para calcular um escore, utilizando as respostas dessas dimensões. Após isso, foi desenvolvido um aplicativo para dispositivo Android, que coleta as respostas do usuário, através de índices pré-estabelecidos, gerando o escore de avaliação, a fim de apresentar ao usuário sua atual condição de saúde. Foi realizado testes de utilização do aplicativo em um total de 10 pessoas, de todas as idades, que responderam ao questionário e visualizaram seu resultado em tela.
