Resumo: Growth hormone (GH) and insulin-like growth factor type I (IGF-I) show synergistic actions on muscle anabolism. We describe a large cohort of subjects with isolated GH deficiency (IGHD), caused by a mutation in the GH-releasing hormone receptor gene, with low serum levels of GH and IGF-I, and marked reduction of stature and lean mass, despite performing high level of physical activities in the daily life, suggesting satisfactory muscle function. The aim of this study was to assess muscular function in these IGHD subjects. For this purpose, 31 IGHD (16 males) and 40 control (20 males) subjects, matched by age and physical activity, were enrolled. It was assessed lean mass by bioelectrical impedance; muscle strength by dynamometry of handgrip, trunk extension and knee extension; myoelectrical activity and muscle fatigue by fractal dimension, and conduction velocity in vastus medialis, rectus femoris, and vastus lateralis muscles, by surface electromyography. IGHD group showed higher knee extension strength corrected for weight (p<0.001) and conduction velocity of the muscles vastus medialis (p=0.002), rectus femoris (p=0.002) and vastus lateralis (p= 0.05), lower lean mass (p<0.001) and myoelectrical activity of the vastus medialis (p<0.002), rectus femoris (p<0.001) and vastus lateralis (p<0.001). There were no difference between the groups in handgrip and trunk extension corrected for weight in all studied muscles. IGHD group showed moderate significant correlation (p=0.021) between lean mass and knee extensor strength, without significant correlations in the other function parameters. In conclusion, IGHD subjects show greater strength of knee extensors, similar handgrip and trunk extension strength and higher resistance to fatigue, suggesting better muscular function than controls.

Os “Anões de Itabaianinha/SE”, possuem uma deficiência isolada do GH (DIGH), causada por uma mutação no gene do receptor do hormônio liberador do GH, com baixos níveis séricos de GH e IGF-I, acentuada redução da estatura e da massa magra, porém desenvolvem suas atividades laborais normalmente, sugerindo função muscular satisfatória. O objetivo deste estudo foi avaliar a função muscular em indivíduos com DIGH. Para tanto, foram avaliados 31 indivíduos DIGH (16 homens/15 mulheres, 46,5±12,4 anos, 125,5±7,6 cm, 38,0±6,9 Kg, IMC: 23,4±3,4 Kg/m2) e 40 indivíduos controles CO (20 Homens/20 mulheres, 43,2±11,2 anos, 66,6±10,7 kg, 166,0±10,3cm, IMC: 24,1±2,8 Kg/m2). Foram avaliados o nível de atividade física (através do IPAQ), massa magra através da bioimpedância, força de preensão manual, extensores do tronco e extensores do joelho em valores absolutos (Kgf) e corrigidos pelo peso corporal através da dinamometria portátil, e atividade mioelétrica (%), fatigabilidade central pela análise da dimensão fractal do sinal (%) e fatigabilidade periférica através da velocidade de condução (Hz), dos músculos vasto medial, vasto lateral e reto femoral através da eletromiografia de superfície. Para comparação entre os grupos foi utilizado o teste t de Student e para a correlação entre as variáveis, o coeficiente de Pearson. O grupo DIGH apresentou maiores valores para força de extensores de joelho corrigida pelo peso corporal (p<0,001) e maior velocidade de condução, a qual estima melhor resistência a fadiga periférica dos músculos vasto medial (p=0,002), reto femoral (p=0,002) e vasto lateral (p=0,05); menor massa magra (p< 0,001) e atividade mioelétrica dos músculos vasto medial (p<0,002), reto femoral (p<0,001) e vasto lateral (p<0,001). Não houve diferenças entre os grupos quanto ao nível de atividade física, força de preensão manual e força de extensores de tronco corrigidos pelo peso corporal e dimensão fractal, a qual estima fadiga central em todos os músculos estudados. Indivíduos DIGH apresentaram correlação significativa moderada (p=0,021) entre massa magra e força de extensores do joelho, sem correlações significativas nos demais parâmetros de função.
