Tratamento minimamente invasivo de papiloma humano oral, por meio de terapia fotodinâmica antimicrobiana (aPDT): relato de caso

Resumo: Human papillomavirus (HPV) is a non-enveloped DNA virus that belongs to the Papillomaviridae family and infects the basal keratinocytes of the squamous epithelium through micro-lesions in the skin and mucosa. Although the etiology of oral papillomas in childhood is related to HPV, its real etiology and mode of transmission are not yet fully elucidated and its prevalence in childhood varies between 12.3% and 48.1%. Surgical excision is considered the gold standard for the treatment of oral HPV. Since HPV-related oral lesions do not respond to topical application or systemic administration of cytotoxic or immunomodulating drugs. On the other hand, antimicrobial photodynamic therapy (aPDT) of these oral lesions has shown promise. Thus, the aim of this study was to report, based on the literature, the minimally invasive treatment of oral human papilloma in an infant patient by means of aPDT. The clinical evolution of this clinical case report suggests that aPDT sessions represent a promising method of minimally invasive treatment in pediatric patients over surgical excision of papillomatous lesions, as they are able to reduce their viral load, their risk of malignant transformation of these lesions and its risk of its recurrence over time.

Embora a etiologia dos papilomas orais na infância esteja relacionada com o HPV, sua real etiologia e modo de transmissão não estão ainda completamente elucidados e sua prevalência na infância varia entre 12,3% e 48,1%. A excisão cirúrgica é considerada o padrão ouro para o tratamento do HPV oral. Uma vez que as lesões orais relacionadas ao HPV não respondem à aplicação tópica ou administração sistêmica de drogas citotóxicas ou imunomoduladoras. Por outro lado, a terapia fotodinâmica antimicrobiana (aPDT) destas lesões orais tem se mostrado promissora. Assim, o objetivo deste estudo foi relatar com base na literatura, o tratamento minimamente invasivo de papiloma humano oral de um paciente infantil por meio de aPDT.
