Resumo: This article is based on an ethnography conducted in the Núcleo Trans and in the Ambulatory Clinic for Trans* People at the Federal University of São Paulo (Unifesp). The text aims to accompany and analyze disputes regarding changes in the International Code of Illnesses (ICD) from ICD-10 to ICD-11. In addition, the study investigates changes in the Diagnostic and Statistical Manual of Mental Disorders (DSM), as well as in the norms of the Brazilian Federal Medical Council (CFM). Based on these debates, the text shows that - even in cases when a diagnosis is supposedly absent, or is refused - diagnoses continue to interpellate the care of healthcare providers, patients, and members of social movements.
