Após ser identificada, representou grande evolução na reabilitação de áreas edêntulas por meio dos implantes dentais. Desde que foram introduzidos na odontologia, os implantes dentais osteointegrados se transformaram numa forma segura de devolver a função mastigatória a pacientes que perderam parcial ou totalmente seus dentes. A taxa de sucesso dos implantes tem se mostrado elevada com o passar dos anos, porém, existem condições sistêmicas e locais de cada paciente que podem interferir na osteointegração, levando a sua perda. Um dos fatores relacionados ao insucesso dos implantes é o tabagismo, pois índices estatisticamente significantes de perda indicam a influência negativa do cigarro sobre implantes osteointegrados. O Cirurgião-Dentista deve orientar os pacientes que o hábito de fumar pode exercer efeitos prejudiciais em terapias que utilizam implantes dentários. Há necessidade de cicatrização e osteointegração favoráveis, diminuindo os índices de insucesso e assegurando melhores resultados. Um protocolo deve ser instituído para o paciente, onde a abstenção do vício antes e após a instalação do implante é recomendada. O presente trabalho objetivou revisar a literatura sobre o impacto do uso do tabaco sobre as taxas de sucesso de implantes osteointegrados, assim como pesquisar as alterações que o tabagismo pode provocar no peri-implante e sugerir um protocolo de suspensão e controle do hábito de fumar para a instalação de implantes osteointegrados.Osseointegration envolves the direct apposition of vital bone to titanium implants, without the interposition of fibrous tissue. The identification of this biological property led to the development of sound techniques for the replacement of partial and total edentulous patients. The use of dental implants showed very high success rate in humans. Nevertheless certain systemic conditions are known to impair this integration. Among these conditions the chronic use of tobacco will impair bone matrix to be laid down into implant surfaces. The therapist involved in osseointegration maneuvers should inform the smoker patients about the impact of this habit in implant success. According the literature it seems that the healing time after implant placement is crucial and must be free of smoke inhalation. The smoker patient should be guided through a protocol where smoking cessation before and after the implant placement is observed. The present literature review focused at the effects of tobacco use in the osseointegration of dental implants and its related success rate.
