Resumo: Objective: to identify the nurses' contributions in the context of matrix support in mental health in primary health care. Method: a qualitative research study, based on the Fourth Generation Assessment, carried out with workers from matrix support centers and reference teams of a municipality in southern Brazil. 84 observation hours of, 15 interviews and two groups of data validation were carried out between December 2018 and February 2019. The analysis was performed using the constant comparative method. Results: nurses contribute with matrix support as they act in the inclusion of the nuclei with the teams, in the exercise of management, leadership and shared care, and in the mediation of the mental health actions in the territory. Conclusion: nurses' leadership is decisive, since by concentrating managerial and assistance functions, they collaborate in the construction of care alternatives for people with mental distress in primary health care.
