Effect of high intensity exercise compared to moderate intensity on the rehabilitation of post acute myocardial infarction patients: a systematic review

Resumo: Aerobic training in patients with Coronary Artery Disease reduces the risk of reinfarction, cardiac mortality and mortality from any cause, however, there are divergences in the literature regarding training intensity. This study aims to investigate the effects of high-intensity aerobic training compared to moderate-intensity training on cardiorespiratory fitness and quality of life in individuals after acute myocardial infarction. The studies were selected for this systematic review from the online databases US National Library of Medicine (MEDLINE / PUBMED), Latin American and Caribbean Literatureon Health Sciences (LILACS), Physiotherapy Evidence Database (PEDro). The descriptors were used: coronary artery disease, rehabilitation, aerobic exercise, resistance exercise, combined exercise, meta-analysis, clinicaltrial. Ofthe 1689 initial articles, 44 were screened and 21 were selected for analysis of the full text. Of these, 12 articles were included. The studies in this review showed methodological quality between reason able and good, based on the analysis of the PEDro Scale. High-intensity aerobic training promotes na increase in cardiorespiratory fitness equal to or greater than that of moderate intensity, however, both training programs seem to improve the quality of life in patients after acute myocardial infarction. Future clinical trials and cohort studies are needed to verify the safety of high-intensity training in long-term rehabilitation programs.

Este estudo visa investigar os efeitos do treinamento aeróbico de alta intensidade comparado ao de moderada intensidade sobre a capacidade cardiorrespiratória e qualidade de vida de indivíduos pós-Infarto Agudo do Miocárdio. Os estudos foram selecionados para esta revisão sistemática a partir das bases de dados online US National Library of Medicine (MEDLINE/PUBMED), Literatura Latino Americana e do Caribe em Ciências da Saúde (LILACS), Physiotherapy Evidence Database (PEDro). Dos 1689 artigos iniciais, 44 foram rastreados e 21 foram selecionados para análise do texto completo. Destes, 12 artigos foram incluídos. Os estudos da presente revisão apresentaram qualidade metodológica entre razoável e boa, com base na análise da Escala PEDro. O treinamento aeróbico de alta intensidade promove incremento do condicionamento cardiorrespiratório igual ou superior ao de moderada intensidade, contudo, ambos os treinamentos parecem melhorar a qualidade de vida em pacientes pós-Infarto Agudo do Miocárdio.
