Resumo: Objective: to identify the impact that knowledge of population's health reality has on students of Medicine course during the curricular internship in Primary Health Care. Methods: this is a consecutive cross-sectional study, carried out through the application of a voluntary questionnaire with closed questions, before and after the internship in Primary Health Care in 10 cities in the central-west region of the state of Rio Grande do Sul. Results: in both stages, most students were female. The answers to the questionnaires showed that, post activity, there was a significant improvement in the perception, on part of the interns, that the conducts used were consistent with the health reality of the assisted population. Regarding the evolution of a preventive attitude on part of the student, there was an impact on activities related to the prevention of diseases during the medical consultation. Final Considerations: after completing the internship in PHC, there is a significant improvement in the student's perception that there was a greater consistency between the medical conducts taken by them and the real health needs of the assisted population. A positive and substantial change was also observed, both in guidelines on preventive attitudes during medical consultations, especially aimed at controlling Noncommunicable Diseases, and in general orientations given to their patients. Thus, it can be considered that carrying out curricular internship activities in PHC is an important component in the final training of undergraduate medical students.
