Atenção à saúde de crianças e adolescentes vivendo com HIV/AIDS: avaliação dos familiares/cuidadores

Resumo: | The objective was to assess the presence and extent of the attributes of primary health care to children and adolescents living with HIV/AIDS, according to the PCATool-Brazil Child version instrument. This dissertation is part of a matrix design, which is a cross-sectional survey, conducted in an outpatient facility for pediatric infectious diseases, with family members/caregivers of children and adolescents living with HIV/AIDS. Data collection was carried out by applying the PCATool-Brazil Child version instrument. The analysis was performed with the software SAS version 9.3. Respected to the ethical aspects in accordance with Resolution 196/96, in force during the study period. The population amounted to 71 family/caregivers, 51% of which children/adolescents were between 13 and 19 years old, 51% lived in other cities, 54% the mother was the primary caregiver and 54% had income below 1000 actual. The clinical profile, 90% of children/adolescents acquired HIV through vertical transmission, 51% was receiving treatment. As regards the use of health services, 94% knew the nearest health service, 95% had a Primary Health Care service (PHC), however, only 44% took the child/adolescent in this service. To evaluate the quality of health care for children and adolescents living with HIV/AIDS were identified as regular source of care PHC service and, mostly, specialized service to HIV (78.8%). The results showed no statistically significant differences between services, as both had satisfactory scores values (mean ≥6,6) for attributes of: First Contact Access - use; First Contact Access - accessibility; Longitudinality; Coordination - integration of care; Coordination - information system; Integrality - available services. On the analysis of attributes, no service (specialized and PHC) showed satisfactory value overall score, but had near ideal (average 6.43 and 6.13, respectively). Furthermore, no sociodemographic variable, clinical and service usage was associated with high overall score. There was no distinction between health services, however emphasize the need to integrate both services aiming to improve care and improvement of its attributes, which implies reformulations in the aspects of structure and performance. The study had a limitation by the size of the population and it was produced in a medium-sized city, and the generalization of data should be made with caution. |

The study had a limitation by the size of the population and it was produced in a medium-sized city, and the generalization of data should be made with caution.Objetivou-se avaliar a presença e a extensão dos atributos da Atenção Primária à Saúde às crianças e aos adolescentes vivendo com HIV/AIDS, segundo o instrumento PCATool-Brasil versão Criança. Essa dissertação faz parte de um projeto matricial, o qual se trata de uma pesquisa transversal, desenvolvida em um serviço ambulatorial de infectologia pediátrica, com os familiares/cuidadores das crianças e dos adolescentes vivendo com HIV/AIDS. A coleta dos dados realizou-se por meio da aplicação do instrumento PCATool-Brasil versão Criança. Foi realizada a análise com o Software SAS versão 9.3. Respeitaram-se os aspectos éticos de acordo com a Resolução 196/96, em vigência no período da pesquisa. A população totalizou em 71 familiares/cuidadores entrevistados, dos quais 51% a criança/adolescente tinha idade entre 13 a 19 anos, 51% residiam em outros municípios, 54% a mãe era o cuidador principal e 54% tinha renda abaixo de 1000 reais. Quanto ao perfil clínico, 90% das crianças/adolescentes adquiriu o HIV por transmissão vertical, 51% fazia tratamento. No que se refere à utilização dos serviços de saúde, 94% conhecia o serviço de saúde mais próximo da residência, sendo 95% um serviço de Atenção Primária à Saúde (APS), entretanto, apenas 44% levava a criança/adolescente neste serviço. Para a avaliação da qualidade da atenção à saúde das crianças e adolescentes vivendo com HIV/AIDS foram apontados como fonte regular de atenção o serviço de APS e, majoritariamente, o serviço especializado ao HIV (78,8%). Os resultados não apresentaram diferenças estatisticamente significativas entre os serviços, visto que ambos apresentaram valores de escores satisfatórios (média ≥6,6) para os atributos: Acesso de Primeiro Contato utilização; Acesso de Primeiro Contato acessibilidade; Longitudinalidade; Coordenação integração dos cuidados; Coordenação sistema de informações; Integralidade serviços disponíveis. Na análise conjunta dos atributos, nenhum serviço (especializado e APS) apresentou valor satisfatório de Escore Geral, porém obtiveram próximo ao ideal (médias 6,43 e 6,13, respectivamente). Além disso, nenhuma variável sociodemográfica, clínica e de utilização do serviço esteve associada ao alto Escore Geral. Não houve distinção entre os serviços de saúde, entretanto ressalta-se a necessidade de integração entre ambos visando à qualificação da atenção e ao aprimoramento de seus atributos, o que implica em reformulações nos aspectos de estrutura e desempenho.
