Ovariossalpingohisterectomia: análise cardiorrespiratória, pressórica, hemogasométrica, inflamatória e álgica, nas abordagens convencional, notes híbrida e notes total

Resumo: Ovarysalpingohysterectomy (OSH) is the most frequently surgical procedure conducted in small animal surgery routine. This procedure may be accomplished by laparotomy or by videosurgery techniques, which reduces tissue trauma. The Natural Orifice Translumenal Endoscopic Surgery (NOTES) is a new videosurgery modality that has been shown promising because it reduces postoperative pain. Aiming at identifying the OSH surgical procedure(s) that offer the fewer trans and postoperative hemodynamic, inflammatory and painful stimuli alterations, three different operative techniques were compared. With this purpose, 21 dogs, divided in three groups, were used for the experiment. In the first group, the OSH was done via celiotomy (CG). The second group was undergone to total NOTES technique (TNG) and the third was undergone to hybrid NOTES technique (HNG). Only HNG developed severe acidosis in the transoperative period. During the pain evaluations, only the conventional group needed transoperative rescue analgesia and presented higher pain levels by the Visual Analog Scale and the Melbourne Scale compared to the remaining groups, demanding postoperative rescue analgesia in 100% of the animals. In contrast, none of the dogs in TNG required postoperative analgesic complementation. It has been observed an increase of catalase levels and of butyrylcholinesterase and acetylcholinesterase activities in the three groups. However, the lipid peroxidation indices were almost imperceptible in TNG. With this methodology, it is possible to state that the OSH by total NOTES technique is the one offering fewer hemodynamic alterations as well as less postoperative pain and oxidative damage.

Essa operação pode ser efetuada por meio de laparotomia ou por diferentes técnicas videocirúrgicas, as quais são associadas com reduzido traumatismo tecidual. A Cirurgia Endoscópica Transluminal através de Orifícios Naturais ou NOTES (sigla consagrada originada da expressão Natural Orifice Trasnlumenal Endoscopic Surgery), é uma nova modalidade da videocirurgia que tem sido promissora por demonstrar reduzida dor pós-operatória. Visando identificar o(s) procedimento(s) cirúrgico(s) de OSH que oferece(m) menores alterações hemodinâmicas, inflamatórias e de estímulos dolorosos trans e pós-operatório, foram comparadas três diferentes técnicas operatórias. Para tanto, foram utilizados 21 cães, alocados em três grupos de igual número. No primeiro grupo, os animais foram submetidos à técnica de OSH por celiotomia (GC). No segundo, por meio da técnica de NOTES total (GNT) e no terceiro, pela técnica de NOTES híbrida (GNH). Apenas o grupo GNH desenvolveu acidose severa no transoperatório. Nas avaliações da dor, apenas o grupo convencional necessitou de analgesia resgate transoperatória e apresentou índices mais elevados na Escala Visual Analógica e Escala de Melboure que nos demais grupos, exigindo analgesia resgate pós-operatória em 100% dos animais. Em contraste, no GNT nenhum dos cães requereu complementação analgésica pós-operatória. Nos três grupos, observou-se aumento da catalase, da atividade da butirilcolinesterase e acetilcolinesterase, contudo a peroxidação lipídica apresentou índices quase imperceptíveis no grupo GNT.
