Idosos em processo demencial e a fonoaudiologia: um estudo de caso

Resumo: Purpose: analyzing verbal expression and interpretation/comprehension and underlying mechanisms (hearing, body balance, voice and orofacial motricity) of two elder women with clinical case of dementia one living in family environment and other in an elderly Long Permanence Institution. Materials and methods: field investigation, transversal, qualitative, presented in case study modality. Took part two elderly one non-institutionalized with Alzheimer Dementia, the other institutionalized with Vascular Dementia, valued according to a speech evaluation script that beholds in integrated way aspects of language, hearing, body balance, voice and orofacial motricity. Oral language was evaluated through interview, narrative of a relevant fact in life and a present one, comments, evocation/interpretation of a proverb, interpretation of syllogism, resolving a daily problem and yet the expression space-time location conditions. The hearing was evaluated by pure tone audiometry, speech audiometry, behavioral tests of hearing processing and Long-Lantency Auditory Evoked Potential (LLAEP); body balance through static balance tests, dynamic and movements coordination and Sensory Organization Test; voice through perceptive-hearing analysis; and orofacial motricity through observation of structures and functions of the stomatognathic system, highlighting swallowing dynamics. Results: obtained the elaboration of a protocol version of speech evaluation that seizes the operation of language, hearing, body balance, voice and orofacial motricity. In the elderly' evaluation were found linguistic-cognitive manifestation more compromised in the institutionalized elderly. The results of the organic-physiological aspects evaluation, in both elderly, matches the natural aging process. Conclusions: the script favors accurate diagnoses; the elderly' organic-physiological conditions were compatible with natural aging process, while linguistic-cognitive conditions present more compromised. This report shows the convenience of speech therapist acting at aging, especially with dementia, as much in family as in institutional context.

This report shows the convenience of speech therapist acting at aging, especially with dementia, as much in family as in institutional context.Coordenação de Aperfeiçoamento de Pessoal de Nível SuperiorObjetivo: analisar a expressão e interpretação/compreensão verbal e seus mecanismos subjacentes (audição, equilíbrio corporal, voz e motricidade orofacial) de duas idosas com quadro clínico de demência uma vivendo em ambiente familiar e outra em uma Instituição de Longa Permanência para Idosos. Materiais e métodos: investigação de campo, transversal, qualitativa, apresentada na modalidade estudo de casos. Participaram duas senhoras - uma não institucionalizada com Demência de Alzheimer, e outra institucionalizada com Demência Vascular, avaliadas segundo um roteiro de avaliação fonoaudiólogica que contempla de modo integrado aspectos da linguageml, audição, equilíbrio corporal, voz e motricidade orofacial. A linguagem oral foi avaliada por meio de entrevista, narrativa de um fato relevante da vida e de um atual, comentários, evocação/interpretação de provérbio, interpretação de silogismo, resolução de um problema do cotidiano e, ainda, a expressão das condições de localização espaço-temporal. Avaliou-se a audição por Audiometria Tonal Liminar, logoaudiometria, testes comportamentais do processamento auditivo e Potencial Evocado Auditivo de Longa Latência; o equilíbrio corporal por meio das provas de equilíbrio estático, dinâmico e de coordenação dos movimentos e do Teste de Organização Sensorial. A voz por meio de análise perceptivoauditiva; e a motricidade orofacial por meio da observação das estruturas e das funções do sistema estomatognático, destacando-se a dinâmica da deglutição. Resultados: obteve-se a elaboração de uma versão protocolar de avaliação fonoaudiológica que apreende o funcionamento da linguagem, da audição, do equilíbrio corporal, da voz e da motricidade orofacial. Na avaliação das idosas constataram-se manifestações linguístico-cognitivas mais comprometidas na idosa institucionalizada. Os resultados da avaliação dos aspectos orgânicofisiológicos, em ambas as idosas, conferem com o processo natural de envelhecimento. Conclusões: o roteiro favorece a produção de diagnósticos precisos; as condições orgânicofisiológicas das idosas foram compatíveis com o processo natural de envelhecimento, enquanto que as linguístico-cognitivas se apresentaram mais comprometidas.
