Prevalência de anemia e hipotireoidismo em gestantes atendidas no pré-natal do Hospital Universitário de Santa Maria - RS (HUSM)

Resumo: A normal pregnancy is associated with anatomical and physiological adjustments that cause marked changes in the mother, including the composition of humoral and formed elements of the blood circulating. Probably at no other stage of the life cycle exists biggest change in the functioning of the human body and form in such a short space of time. Thus, the objective of this study was to evaluate the prevalence of anemia and hypothyroidism in pregnant women who attended antenatal care at the hospital of Santa Maria University (HUSM) in the period from July to December 2011. Were performed by the Laboratory of Clinical Analyses of HUSM, the TSH and T4L serum of pregnant women, along with analysis of test results of blood count blood. The results of hematological parameters revealed that the rate of hemoglobin, among 124 pregnant women evaluated, was altered in 16.2%. Another variable was significant hematologic evaluation of the Mean Corpuscular Hemoglobin Concentration (MCHC), which showed changes in 16.4% of 122 pregnant women tested. Hematocrit a total of 61 patients, we observed that 34.4% of them were changed. These results, when assessed on a whole, showed a strong correlation with the presence of anemia. As to possible malfunctions thyroid gland, TSH levels ranged between 0.6 and 1.13 ng / dL, while the fluctuations of FT4 were 0.377 to 6.36 mIU / L. When framed in the classification of clinical hypothyroidism (declared) and subclinical analysis revealed 4 cases of subclinical hypothyroidism, with a prevalence of about 3%, and no cases of clinical hypothyroidism in 116 pregnant women analyzed. The age of the patients ranged from 16 to 44 years and gestational age between 7-38 weeks. Based on finding of a 4% prevalence of elevated TSH during pregnancy, the authors consider important the inclusion of thyroid function laboratory evaluation in the routine prenatal examination. Further studies appear necessary to establish at what gestational age thyroid function evaluation should be started in pregnant women and how frequently it should be repeated during the course of gestation.

Provavelmente, em nenhuma outra fase do ciclo vital exista maior mudança no funcionamento e forma do corpo humano em tão curto espaço de tempo. Desta forma, o objetivo do presente trabalho foi avaliar a prevalência de anemia e hipotireoidismo em gestantes atendidas no pré-natal do hospital Universitário de santa Maria - RS (HUSM), no período de julho a dezembro de 2011. Foram realizadas, pelo Laboratório de Análises Clínicas do HUSM, as dosagens de TSH e T4L no soro das gestantes, juntamente com análise de resultados de exames de hemograma sanguíneos. Os resultados dos parâmetros hematológicos revelaram que o índice de hemoglobina, dentre 124 gestantes avaliadas, estava alterado em 16,2%. Outra variável hematológica significativa foi na avaliação da Concentração de Hemoglobina Corpuscular Média (CHCM), que revelou alteração em 16,4%, das 122 gestantes analisadas. No hematócrito de um total de 61 gestantes, observou-se que 34,4% delas encontravam-se alteradas. Esses resultados, quando avaliados em um todo, mostraram forte correlação com a presença de anemia. Quanto às possíveis disfunções glandulares tireoidianas, os níveis de TSH variaram entre 0,6 e 1,13 ng/dL, enquanto que as flutuações de T4L foram de 0,377 a 6,36 mUI/L. Quando enquadradas na classificação de hipotireoidismo clínico (declarado) e subclínico, foram observados 4 casos de hipotireoidismo subclínico, com prevalência em torno de 3%, e nenhum caso de hipotireoidismo clínico em 116 gestantes analisadas. A idade das gestantes variou entre 16 a 44 anos e o tempo gestacional entre 7 a 38 semanas. Com base nos resultados obtidos, considera-se de grande importância o contínuo monitoramento da presença de anemia em gestantes e torna-se cada vez mais imprescindível a inclusão da avaliação tireoidiana na rotina no exame pré-natal.
