Resumo: Background: The intrinsic and extrinsic skin aging present biomechanical and morphological differences, which are reflected in the appearance of roughness, superficial and deep wrinkles, atrophy, reduced elasticity, hypo- and hyperpigmentation and actinic keratosis. The photodamage causes accentuation of all these features and increases the risk for development of premalignant lesions (actinic keratosis) and malignant (basal and squamous cell carcinomas). Objective: to evaluate and compare the characteristics of the flexor (with a predominance of intrinsic aging) and extensor (chronically exposed to sunlight area and other environmental factors, with a predominance of extrinsic aging or photoaging) skin surfaces of the forearms. Methods: The cross-sectional study of diagnostic interventions, including 23 females, aged over 60 years. The extensor and flexor faces of right and left forearms were compared in relation to clinical parameters and non-invasive instrumental measures, such as: skin surface (profilometry), viscoelastic properties, hydration (corneometry) as well as dermal thickness and echogenicity (ultrasound). Results: The water content in the corneal layer showed a significant difference as the flexor area showed an average value greater than the extensor face. The average of the measurements obtained by high-frequency ultrasound showed difference between the faces. The skin surface measurements showed significant differences when compared roughness and desquamation of the extensor and flexor faces on both forearms, but for the flexor face, the differences were smaller. Regarding the wrinkles depth, there was a significant difference of average between the evaluated faces only for the left forearm (p = 0.017), once the flexor face presented an average value greater than the extensor. There was no significant correlation between measurements of the aqueous content of the corneal layer and the viscoelastic properties of the skin, as well as among those obtained by ultrasound. In the prophylometry, only the scaling presented a negative correlation with the corneometry. The visco-elastic properties of the skin showed a positive correlation between the faces. The measurements obtained by the ultrasound and the visco-elastic properties of the skin showed a positive correlation in the flexor face. In the extensor face, there was a positive correlation between roughness, scaliness and wrinkle depth. In the flexor face, the depth of the wrinkles presented negative correlation with a dermal thickness. Dermal density showed negative correlation with other ultrasound measurements. These results were in agreement with established clinical parameters such as wrinkles, elastosis, decreased elasticity and water content, demonstrating the differences between photoaging and chronological aging. Conclusion: Significant differences in the biophysical characteristics of the extensor and flexor surfaces of the forearms were detected. The non-invasive instrumental measures were positively correlated with clinical findings. We concluded that they are useful tools for the evaluation of photoaging and chronological aging in clinical research, as efficacy and safety parameters.

A redução da espessura da epiderme é decorrente do tempo aumentado de migração das células basais até a camada córnea e da descamação pela diminuição do conteúdo hídrico e dos componentes do NMF (Natural Moisturizing Factor ou Fator Natural de Hidratação). A menor densidade do tecido colágeno dérmico deve-se à redução da sua síntese e aumento da degradação. O fotodano, relacionado à exposição solar crônica, acentua todas estas características e pode acarretar o desenvolvimento de lesões pré-malignas (queratoses actínicas) e malignas (carcinomas baso e espinocelular). Objetivo: Avaliar e comparar as características da pele das faces flexora (área coberta, com predomínio do envelhecimento intrínseco) e extensora (área exposta ao sol e outros fatores ambientais, com predomínio do envelhecimento extrínseco ou fotoenvelhecimento) dos antebraços através de parâmetros clínicos e medidas instrumentais não invasivas. Casuística e Métodos: O estudo transversal de intervenção diagnóstica incluiu 23 mulheres com idade superior a 60 anos. As faces extensora e flexora dos antebraços direito e esquerdo foram comparadas em relação aos parâmetros clínicos e as medidas instrumentais não invasivas, tais como: hidratação (corneometria), relevo da superfície da pele (profilometria), propriedades viscoelásticas, espessura dérmica e ecogenicidade dérmicas (ultrassom). Resultados: O conteúdo aquoso da camada córnea mostrou diferença significante, ou seja, a face flexora (mais protegida) apresentou um valor médio maior do que a extensora (menos protegida). A média das medidas obtidas através das imagens do ultrassom de alta frequência mostrou diferenças entre as faces; a flexora apresentou menor espessura dérmica e maior ecogenecidade comparada à extensora. As medidas do relevo da pele mostraram diferenças significantes quando comparadas aspereza e descamação entre as faces extensora e flexora em ambos os antebraços, sendo menores na face flexora. Com relação à profundidade das rugas, houve diferença significante da média entre as faces avaliadas apenas para o antebraço esquerdo (p=0,017), uma vez que a face flexora apresentou média maior do que a extensora. Não houve correlação significativa entre o conteúdo aquoso da camada córnea (corneometria) e das propriedades visco-elásticas da pele, assim como entre elas e as medidas obtidas pelo ultrassom. Na profilometria, apenas a descamação apresentou correlação negativa com a corneometria. As medidas das propriedades visco-elásticas da pele apresentaram correlação positiva entre as faces. As medidas obtidas pelo ultrassom e as das propriedades visco-elásticas da pele apresentaram correlação positiva na face flexora. Na face extensora, houve correlação positiva entre aspereza, descamação e profundidade das rugas. Na face flexora, a profundidade das rugas apresentou correlação negativa com a espessura dérmica; a densidade dérmica apresentou correlação negativa com as outras medidas do ultrassom. Estes resultados paralelos aos parâmetros clínicos estabelecidos como rugas, elastose e diminuição da elasticidade e do conteúdo hídrico, evidenciam as diferenças entre fotoenvelhecimento e envelhecimento intrínseco. Conclusão: Diferenças significativas nas características biofísicas das faces extensora e flexora dos antebraços, ou seja, áreas com predominância de fotoenvelhecimento ou envelhecimento intrínseco, respectivamente, foram detectadas.
