Relação da densidade tumoral das células Dendríticas e Natural Killer com os aspectos clínico-patológicos e prognóstico no adenocarcinoma gástrico

Resumo: Introduction: Dendritic (DC) and Natural Killer (NK) cells are responsible for individual’s natural immunity and more and more its presence infiltrating the tumors is being related with clinical and pathological features in different types of tumors, including Gastric Cancer. Objectives: Evaluate the density of dendritic and natural killer cells in gastric adenocarcinoma and its relation with several prognostic factors and with survival. Methods: We studied 72 cases of gastric adenocarcinoma submitted to gastrectomy with D2 lymph node dissection in the Service of General Surgery of The University of Santo Amaro, between 1997 and 2007. We analyzed age, gender, tumor location in the stomach, degree of differentiation (well, moderately or poorly differentiated types), Laurén’s classification, tumor stage, lymph node invasion, metastasis and survival. The evaluation of dendritic cells was done through immunohistochemical evaluation with CD1a and CD57. Ten high power (400x) fields (HPF) were analyzed and the patients divided into 2 groups: those with high cell density (> 15 cells/10 HPF) and those with low cell density (< 15 cells/10 HPF). Results: Regarding the density of DC there was a very small percentage of patients (5%) with high density; this finding did not allow a statistical evaluation. There was no correlation of NK with the several findings studied: gender, age, location, tumor depth (T), lymph node invasion (N), metastasis (M) and cell differentiation. In more advanced stages the NK cells density was significantly higher (p<0.02). The survival was higher in the group with high density of NK when compared with the low-density group (32.25 versus 18.22 months) (p=0.0007). Conclusions: The analysis of the several clinical, surgical and pathological findings did not show difference in Natural Killer cells density. Only in advanced stages the density of NK cells was significantly higher. Patients with high NK cells density presented higher survival rates when compared to those with low density.

Patients with high NK cells density presented higher survival rates when compared to those with low density.Introducao: Celulas Dendriticas (CD) e celulas Natural Killer (NK) sao responsaveis pela imunidade natural do individuo e, cada vez mais, suas presencas no infiltrado tumoral estao sendo relacionadas com alteracoes clinico-patologicas de varios tumores, inclusive no Cancer Gastrico. Objetivo: Avaliar a densidade de celulas Dendriticas e de celulas Natural Killer no adenocarcinoma gastrico, sua eventual correlacao com os diversos fatores prognosticos e sobrevivencia. Metodos: Foram estudados 72 casos de adenocarcinoma gastrico submetidos a gastrectomia com linfadenectomia a D2 no servico de Cirurgia Geral da FMUNISA no periodo de 1997 a 2007. Foram analisados: genero, idade, localizacao do tumor no estomago, grau de diferenciacao celular, classificacao de Lauren, estadio, disseminacao para linfonodos, metastases e sobrevivencia. A avaliacao das celulas CD e das celulas NK foram realizadas por imuno-histoquimica com os reagentes CD1a e CD57. Foram analisadas 10 campos de grande aumento (CGA) (400x), e os doentes foram divididos em 2 grupos: doentes com Alta densidade de celulas (.15 celulas /10 CGA e doentes com Baixa densidade (< 15 celulas CGA). Resultados: Com relacao a densidade de CD houve um percentual pequeno de doentes (5%) com Alta densidade, nao sendo possivel realizar a analise estatistica. Referente as celulas NK nao houve correlacao com diversas variaveis estudadas: genero, idade, localizacao, profundidade (T), invasao linfonodal (N), metastases (M), diferenciacao celular (pouco, moderadamente ou bem diferenciado). Quanto ao estadio houve significancia (p<0,02) quando relacionado a Alta densidade de NK aos estadios mais avancados. A sobrevivencia foi maior (p=0,007) no grupo de Alta densidade de celulas NK, media de 32,25 meses, contra uma media de 18,22 no grupo de Baixa densidade. Conclusoes: A densidade de celulas NK nao difere entre as variaveis clinico-patologicas, com excecao do estadiamento, onde nota-se um predominio de Alta densidade nos estadios TARDIO. Doentes com Alta densidade de celulas NK apresentaram uma maior sobrevivencia tanto, global,quanto no estadio TARDIO, quando comparados aos de Baixa densidade.
