Resumo: | Objectives: The purpose of this thesis was to contribute to quality improvement of multidose eyedrops by analyzing factors related to appropriate eyedrop packaging, identifying safety issues and user perceptions of eyedrop usability, assessing drop volume variability and the impact of eyedrop bottle tip geometry and instillation angle on contamination rates, with the aim of encouraging specification of requirements for topical ophthalmic products. Methods: From the identification of the multidose eyedrops marketed in Brazil, 297 packages and package inserts were analyzed for informational content, alongside an analysis of the variability of the drop volume and the correlation of the mass per drop measured in five samples of five lubricant eyedrop brands and an evaluation of the performance of five samples of six eyedrops on an airplane during varying altitudes. Risk of cross contamination of six samples from nine brands of multidose eyedrops that were contaminated exclusively on the exterior side of rounded versus sharp tips was measured at instillation angles of 45° and 90° degrees. A survey aiming at a qualitative analysis of the perception, knowledge, usability and consumption trends of eyedrop was administered to 1.003 consumers. Results: Eyedrops represent 91.6% of ophthalmic drugs currently sold in Brazil. The mean drop volume reported by the manufacturers was 36.9 ± 7 µL, ranging from 25.6 to 71.4 µL. Instructions for using eyedrops are uneven and not illustrative, and some products were found to have inadequate package inserts. In the performance test at high altitude, one brand presented leakage and an irregular efflux of drops in all samples. An increased risk of infection transmission was observed in vials with a rounded tip versus a sharp tip [69.4% vs. 22.2%, P=0.001], especially when inclined at an instillation angle of 45° versus 90° [83.3% vs. 44.4%, P=0.005]. Among the 1,003 survey respondents, 84.6% indicated experiencing a part of the eyedrop running over the face upon instillation and 76.2% preferred to have information regarding the total number of drops contained in the eyedrop bottles. Conclusions: The data indicate a possible inadequacy and lack of standardization of eyedrop packaging and bottles, which yield drops of excess volume for the human eye. Moreover, eyedrop bottles with perpendicular platform tips showed greater safety than those with rounded nozzles. Manufacturers should inform the total number of drops in each product, in addition to total net volume. A review of user instructions and requirements for quality standards for the registration of ophthalmic products is recommended. |

A review of user instructions and requirements for quality standards for the registration of ophthalmic products is recommended.Objetivos: Esta tese foi realizada com o objetivo de contribuir com a melhoria da qualidade dos colírios multidose mediante a análise de parâmetros referentes à adequação das embalagens e a identificação de questões de segurança e percepções do usuário quanto à sua usabilidade, avaliando a variabilidade do volume das gotas e o impacto da geometria do bico e do ângulo de instilação nas taxas de contaminação, de forma a incentivar a especificação de requisitos para os produtos oftálmicos de uso tópico. Métodos: A partir da identificação dos colírios multidose comercializados no Brasil, foram realizadas: análise das informações contidas nas bulas e embalagens de 297 colírios; análise da variabilidade do volume da gota e da correlação da massa por gota em cinco amostras de cinco colírios lubrificantes; avaliação do desempenho de cinco frascos de seis marcas de colírios em aeronaves durante o voo; avaliação do risco de contaminação cruzada de acordo com a inclinação do frasco e a geometria do batoque de seis frascos multidose de nove marcas de colírios contaminados na face externa do seu bico; e análise qualitativa da percepção, conhecimento, modo de utilizar e tendências de consumo por meio de um questionário respondido por 1.003 usuários. Resultados: Os colírios constituem 91,6% dos medicamentos para a via oftálmica comercializadas no Brasil atualmente. O volume das gotas informado pelos fabricantes foi em média 36,9 ± 7 µL, variando de 25,6 a 71,4 µL. As instruções de uso dos colírios são desiguais e pouco ilustrativas, e verificou-se uma inadequação na bula de alguns produtos. No teste de desempenho durante o voo, uma das marcas apresentou vazamento com efluxo irregular de gotas em todas as amostras. Observou-se um risco aumentado de transmissão de infecções nos frascos que apresentam o bico de formato arredondado em relação aos de plataforma perpendicular [69,4% vs. 22,2%; P=0.001], principalmente quando utilizados de forma inclinada a 45° em vez de invertidos a 90° [83,3% vs. 44,4%; P=0.005]. Dos 1.003 respondentes do questionário, 84,6% com alguma frequência percebem parte da gota escorrer sobre o rosto após sua instilação e 76,2% concordaram de alguma forma que gostariam de ter informações sobre a quantidade total de gotas dos frascos dos colírios. Conclusões: Identificou-se uma possível inadequação das embalagens multidose dos colírios, que apresentam gotas de volume excessivo ao olho humano. Os colírios com bicos de plataforma perpendicular mostraram maior segurança do que os arredondados. Os fabricantes deveriam informar o número total de gotas de cada frasco, além do seu volume líquido total.
