Evidências de validade e confiabilidade da escala de avaliação multidimensional-MIDAS em paciente pós infarto agudo do miocárdio

Resumo: Introduction: Quality of life (QOL) is the individual's perception of their position in life, considered in the context of culture, values in which they live, and elaborates their goals, expectations, standards and concerns. To evaluate this condition is very important determine the effectiveness of an intervention. Objective: To evaluate the reliability and validity of MIDAS multidimensional rating scale in measuring the quality of life of patients after myocardial infarction. Simultaneously, to correlate questionnaire domains with risk factors for cardiovascular disease, assessing possible difference between female and male. Methodology: This is a descriptive cross-sectional study with a quantitative approach, divided into two stages: the first to test the psychometric proprieties, and the second to inquiry the quality of life. The reliability was assessed by internal consistency and inter and intra observer reproducibility in hospitalized patients and outpatients. Similarly, it was also studied convergent and predictive validity. To assess the QoL was used a sample categorization tool and MIDAS. Results: In the evaluation of the psychometric properties participated 83 patients in which found satisfactorily validity and reliability of the instrument. In the second stage of the study, 273 subjects were recruited. The average age of the sample was 63.6 years; men were 67% and Caucasian 42.5%and the majority had low level of education (69.9%) and was married (67.8%). The domains with the lowest average in relation to quality of life were the addiction, medication and physical activity. The domains that had the best scores were side effects and diet. There was a significant difference to the worst QoL scores between male and female. Conclusion: MIDAS’ questionnaire has significant evidence to reliability and validity in hospitalized patients and outpatients after myocardial infarction in this Brazilian sample. Post heart attack patients had impairments in the quality of life. In this regard, the dependence domain was the most affected. The maintenance of risk factors was significant even after AMI with difference between the sexes.

Avaliar é de suma importância para determinar a eficácia das intervenções. Objetivo: Avaliar a confiabilidade e validade da escala multidimensional-MIDAS, na qualidade de vida dos pacientes após IAM, e correlacionar os domínios com os fatores de risco para doenças cardiovasculares, aferindo a diferença entre os sexos feminino e masculino. Metodologia:Trata-se de um estudo descritivo de corte transversal com abordagem quantitativa dividido em duas etapas: a primeira para testar a psicometria; a segunda para avaliar a qualidade de vida. Para medir a confiabilidade foi verificada a consistência interna e a reprodutibilidade inter e intra observador, diferenciando o ambiente hospitalar e ambulatorial. Também foi constatada a validade convergente e preditiva. Para avaliar a QV foi utilizado instrumento de categorização da amostra e o MIDAS. Resultados: Na verificação das propriedades psicométricas participaram 83 pacientes, no qual se constatou de forma satisfatória a manutenção da validade e da confiabilidade do instrumento. Na segunda etapa participaram estudo um total de 273 sujeitos. A idade média na amostra foi de 63,6 anos. A maioria era do sexo masculino (67%) e etnia branca (42,5%), apresentando baixo nível de escolaridade (69,9% não possuem o 1º grau completo) e casados (67,8%). Os domínios com pior média em relação a qualidade de vida foram dependência, Medicação e Atividade física. Os domínios que tiveram melhores escores foram efeitos Colaterais e Dieta. Observou-se diferença significativa para os piores escores de QV entre o sexo masculino e feminino. Conclusão: O MIDAS possui evidências de confiabilidade e validade para ser utilizado no ambiente hospitalar ambulatorial em paciente pós IAM nessa amostra de pacientes brasileiros. Os pacientes pós infarto tiveram prejuízos na qualidade de vida com o domínio de dependência sendo o mais afetado.
