Alterações eletrocardiográficas na hipotermia acidental em região urbana e tropical

Resumo: | Hypothermia is defined as the condition in which the core temperature (rectal, esophageal or tympanic) reaches values below 36°C. This may be accidental, metabolic or therapeutic. The accidental form is frequent in cold-climate countries, being rare in those with a tropical or subtropical climate. Purpose: The aim of this study was to evaluate electrocardiographic changes of patients with accidental hypothermia. Methods: In 59 patients with hypothermia the following electrocardiogram (ECG) parameters were analyzed: rhythm and heart rate, P wave characteristics, PR interval, duration of the QRS complex, presence of J wave and its characteristics, and correlation with the degree of hypothermia, changes in T wave regarding its polarity and characteristics, duration of the QT interval corrected for heart rate using Bazett’s and Fridericia`s formula, and the eventual presence of both supraventricular or ventricular arrhythmias. Results: In six patients the ECG was normal. Sinus bradycardia was observed in 54,2% of the patients. J wave was present in 51 patients and its voltage correlated inversely and statistically significantly with the core temperature. Changes in T wave were observed in 47.4% of the cases. The QT interval, adjusted corrected for heart rate by Bazett’s formula was prolonged in 74,5%, and by Fridericia’s formula in 71,1% of the cases. Idioventricular rhythm was found in six cases, total atrioventricular block in three and junctional rhythm and atrial fibrillation in two patients. Conclusions: ECG changes in accidental hypothermia are frequent and characteristic for this entity improving diagnosis in unconscious patients. |

Pela sua etiologia a hipotermia pode ser acidental, metabo1ica ou terapeutica. A forma acidental e frequente em paises de clima frio, sendo rara nos de clima tropical ou subtropical. Objetivo: Analisar as alteracoes eletrocardiograficas de 59 pacientes internados na unidade de terapia intensiva em hospital de emergencia na cidade de São Paulo (Brasil) com hipotermia acidental. Metodos: Os eletrocardiogramas foram analisados de forma cega e avaliados: o ritmo e frequencia cardiaca, a onda P, duracao do intervalo PR, duracao do complexo QRS, a presenca da onda J e suas caracteristicas de localizacao, polaridade, voltagem, aspecto e a correlacao com grau de hipotermia, alteracoes da onda T quanta a sua polaridade e caracteristicas, a duracao do intervalo QT corrigido para a frequencia cardiaca, e a eventual presenca de arritmias. Resultados: Em seis pacientes o eletrocardiograma foi resultou normal. A bradicardia sinusal foi observada em 54,2 por cento dos pacientes. A onda J, que e uma deflexao positiva extra de inscricao lenta entre o fim do complexo QRS e o inicio do segmento ST esteve presente em 51 pacientes e sua voltagem teve correlacao inversa e estatisticamente significante com a temperatura central. Alteracoes da onda T foram observadas em 47,4 por cento dos casos. 0 intervalo QT corrigido para a frequencia cardiaca pela formula de Sazett esteve prolongado em 74,5 por cento dos pacientes e pela formula de Fridericia em 71,1 por cento. 0 ritmo idioventricular foi encontrado em seis casos, bloqueio atrio ventricular total em tres e ritmo juncional e fibrilacao atrial em dois pacientes.
