Resumo: Introduction: Colorectal cancer is one of the most prevalent cancer worldwide. In recent decades, the study of specific biomarkers aiming to a personal therapy has been the scene of several studies. The study of omics, more recently lipidomics has the purpose of analyze the individual lipids, which provides data to better understanding of human lipidome. The evolution of mass spectrometry methods such as technology for MALDI-MS, enabled the detection and identification of a wide variety of lipids with great potential to open new avenues for predictive and preventive medicine. Objective: To determine the lipidomic profile of patients with colorectal cancer in stages I, II and III, of patients with polyps and a control group. Compare the profiles of cancer patients in the preoperative to the postoperative. Patients and methods: We selected 107 patients, 40 with colorectal cancer prior to surgical resection, 23 patients after surgical resection, 12 patients with adenomatous polyp and 32 patients with normal colonoscopy. All patients were subjected to samples of peripheral blood, it was subsequently carried out extraction of the lipid from the plasma. The samples were analyzed by MALDI-MS technique for identifying lipids. Results: Using the method of analysis of variance (ANOVA) revealed differences between groups cancer and controls as well as polyps and controls. The Ion 810.1 was the one that best differentiated the groups, with p <0.01 and accuracy of 85% by PLSDA method. Among the 15 VIP lipids VIP, eight have been identified as potential biomarkers. The polyketide (810.1) was the hypo-represented lipid in cancer and over-represented in the polyp and control. The lipid profile of individuals with cancer compared to the same group after resection had an accuracy of 66% by PLSDA test. However when comparing the control patients to the cancer patients various lipid profiles had been observed with an accuracy of 100% for PLSDA test. The comparison of the polyp group to controls, by the PLSDA method, although it had not demonstrated difference between the groups, resulted in an accuracy of 74%. Conclusions: We identified possible lipid biomarkers among cancer patients compared with control groups and polyps. The polyketide lipid (810.1) was the best biomarker to differentiate cancer to the control group and polyps. We found no difference between the biomarkers in preoperative group compared to the postoperative as in the polyp group compared to the control.

Nas últimas décadas, o estudo de biomarcadores específicos objetivando uma terapia indidualizada tem sido o palco de diversas pesquisas. O estudo das ômicas, mais recentemente a lipidômica, tem a finalidade de analisar os lipídeos individuais, a qual fornece dados para melhor compreensão do lipidoma humano. A evolução dos métodos de espectometria de massa, como a tecnologia por MALDI-MS, possibilitou a detecção e a identificação de uma ampla variedade de lipídeos com grande potencial para abrir novos caminhos para a medicina preditiva e preventiva. Objetivo: determinar o perfil lipidômico de pacientes com câncer colorretal em estádios I, II e III, dos pacientes com pólipos e de um grupo controle. Comparar os perfis dos pacientes no pré-operatório com os do pós-operatório. Casuística e método: Foram selecionados 107 pacientes, 40 com câncer colorretal, antes da ressecção cirúrgica, 23 pacientes após ressecção cirúrgica, 12 pacientes com pólipo adenomatoso e 32 pacientes com colonoscopia normal. Todos os pacientes foram submetidos a coleta do sangue periférico, posteriormente foi realizada a extração do lipídeo a partir do plasma. As amostras foram analisadas por técnica de MALDI-MS para a identificação dos lipídeos. Resultados: utilizando o método de análise de variância (ANOVA) observou-se diferenças entre os grupos câncer e controle, assim como entre câncer e pólipo. O íon 810,16354 foi o que melhor diferenciou os grupos, com p< 0,01 e acurácia de 85% pelo método de PLSDA. Entre os 15 lipídeos Vips, foram identificados oitos como potenciais biomarcadores. O policetídeo (810,1) foi o lipídeo hipo-representado no câncer e hiper-representado no pólipo e controle. O perfil lipídico dos indivíduos com câncer comparado ao mesmo grupo após ressecção teve uma acurácia de 66% pelo teste de PLSDA. Entretanto quando comparamos os pacientes do grupo controle aos pacientes com câncer observamos perfis lipídicos distintos com acurácia de 100% pelo teste de PLSDA. A comparação do grupo pólipo ao controle, embora não tenha demonstrado separação entre os grupos, o método de PLSDA obteve acurácia de 74% entre os grupos. Conclusões: Foram identificados possíveis biomarcadores lipidicos entre os pacientes com câncer comparados aos grupos controle e pólipo. O lipídeo policetídeo (810,1) foi o melhor biomarcador para diferenciar o grupo câncer do controle e pólipo. Não encontramos diferença entre os biomarcadores no grupo pré comparado ao pós-operatório assim como no grupo pólipo em relação ao controle. 1. Câncer Colorretal 2.
