Avaliação da aterosclerose nas artérias ilíacas de pacientes autopsiados

Resumo: Introdução: a doença cardiovascular aterosclerótica nas artérias ilíacas ocorre de maneira assintomática e está associada a aterosclerose sistêmica, o que contribui para os elevados índices de mortalidade. Objetivos: foi avaliar a intensidade macroscópica e microscópica da aterosclerose; mensurar a espessura das camadas íntima e média; quantificar a porcentagem de depósito lipídico e de fibras colágenas, de artérias ilíacas direita e esquerda, e associar esses dados a fatores de risco como envelhecimento; sexo e causa de morte. Métodos: foram obtidos 202 fragmentos de artérias ilíacas direita e esquerda de pacientes autopsiados. Para a classificação da intensidade da aterosclerose foi considerada a extensão de estrias lipídicas, de placas ateromatosas, de fibrose e de calcificação. Na macroscopia, por meio de uma escala de 12,0cm foi registrado um ponto correspondente a intensidade da aterosclerose. A classificação foi discreta, de 0,1cm a 4,0cm; moderada, de 4,1cm a 7,0cm e acentuada de 7,1cm a 12,0cm. Na microscopia, os fragmentos foram corados por hematoxilina e eosina e analisados ao microscópio de luz, sendo o acometimento discreto, quando menor que 25%; moderado, entre 26% a 50% e acentuado quando acima de 50%. Para a mensuração da espessura das camadas íntima e média foi feita a coloração por Verhoeff e para o depósito lipídico, o Sudan Vermelho. Ambas as análises foram feitas no software Axion Vision®. A porcentagem de fibras colágenas foi realizada no software Leica Qwin Plus®, sendo as lâminas coradas pelo Picrossírius. Para análise estatística foi utilizado o software graphPad Prism®. Resultados: a intensidade macroscópica e microscópica da aterosclerose; o espessamento das camadas íntima e média; a porcentagem de depósito lipídico e de fibras colágenas se correlacionaram de forma positiva e significativa com a idade, sendo significativamente maiores no sexo masculino e na causa de morte cardiovascular. Conclusão: os diferentes métodos usados fornecem novas aplicações para uma avaliação mais sensível dos grupos de risco e das alterações precoces associadas à aterosclerose pré-clínica nas artérias ilíacas.

Objectives: to evaluate the macroscopic and microscopic intensity of atherosclerosis; measure the thickness of the intima and middle layers; quantify the percentage of lipid deposits and collagen fibers, right and left iliac arteries, and associate these data with risk factors such as aging; sex and cause of death. Methods: 202 fragments of right and left iliac arteries were obtained from autopsied patients. For the classification of the intensity of atherosclerosis, we considered the extension of lipid streaks, atheromatous plaques, fibrosis and calcification. In macroscopy, a point corresponding to the intensity of atherosclerosis was recorded using a 12.0cm scale. In order to measure the thickness of the intima and medium layers, Verhoeff staining and the Red Sudan lipid deposit were made. Both analyzes were done in Axion Vision® software. The percentage of collagen fibers was performed in the Leica Qwin Plus® software, with the blades stained by Picrossírius. For statistical analysis, graphPad Prism® software was used. Results: the macroscopic and microscopic intensity of atherosclerosis; thickening of the intima and middle layers; the percentage of lipid deposits and collagen fibers correlated positively and significantly with age, being significantly higher in males and in the cause of cardiovascular death. description Introdução: a doença cardiovascular aterosclerótica nas artérias ilíacas ocorre de maneira assintomática e está associada a aterosclerose sistêmica, o que contribui para os elevados índices de mortalidade. Objetivos: foi avaliar a intensidade macroscópica e microscópica da aterosclerose; mensurar a espessura das camadas íntima e média; quantificar a porcentagem de depósito lipídico e de fibras colágenas, de artérias ilíacas direita e esquerda, e associar esses dados a fatores de risco como envelhecimento; sexo e causa de morte. Métodos: foram obtidos 202 fragmentos de artérias ilíacas direita e esquerda de pacientes autopsiados. Para a classificação da intensidade da aterosclerose foi considerada a extensão de estrias lipídicas, de placas ateromatosas, de fibrose e de calcificação. Na macroscopia, por meio de uma escala de 12,0cm foi registrado um ponto correspondente a intensidade da aterosclerose. A classificação foi discreta, de 0,1cm a 4,0cm; moderada, de 4,1cm a 7,0cm e acentuada de 7,1cm a 12,0cm. Na microscopia, os fragmentos foram corados por hematoxilina e eosina e analisados ao microscópio de luz, sendo o acometimento discreto, quando menor que 25%; moderado, entre 26% a 50% e acentuado quando acima de 50%. Para a mensuração da espessura das camadas íntima e média foi feita a coloração por Verhoeff e para o depósito lipídico, o Sudan Vermelho. Ambas as análises foram feitas no software Axion Vision®. A porcentagem de fibras colágenas foi realizada no software Leica Qwin Plus®, sendo as lâminas coradas pelo Picrossírius. Para análise estatística foi utilizado o software graphPad Prism®. Resultados: a intensidade macroscópica e microscópica da aterosclerose; o espessamento das camadas íntima e média; a porcentagem de depósito lipídico e de fibras colágenas se correlacionaram de forma positiva e significativa com a idade, sendo significativamente maiores no sexo masculino e na causa de morte cardiovascular. Conclusão: os diferentes métodos usados fornecem novas aplicações para uma avaliação mais sensível dos grupos de risco e das alterações precoces associadas à aterosclerose pré-clínica nas artérias ilíacas.
