LOCATION DYNAMICS OF HEALTH SERVICES: EVIDENCE FOR PERNAMBUCO (2007 and 2019)

Resumo: | This article presents an analysis of the changes in the dynamics of the spatial distribution of Health establishments in the state of Pernambuco, in the years 2007 and 2019. Data from the companies were collected, georeferenced and mapped from the addresses listed in the National Register of Health Establishments (CNES). The results indicated that from 2007 to 2019 there were significant changes in the spatial distribution of private and public health establishments: private units, originally more concentrated than public ones, showed a drop in concentration indicators and started to have a spatial distribution more like public establishments. The estimates also suggest the occurrence of a deconcentration of the more complex health services, a fact that is consistent with the process of interiorization of Higher Education in Health, associated with improvements in the state's transport infrastructure, in addition to the increase in family income in medium and small cities. Thus, the results can help in the formulation and application of public policies aimed at improving access to health and intervene in the improvement of demand and service supply networks. It is essential, in this regard, investments and research that focus on the spatial mobility of consumers and health professionals, as well as on supporting better working conditions for these professionals. |

A partir de las direcciones inscritas en el Registro Nacional de Establecimientos de Salud (CNES), todas las respectivas empresas fueron debidamente georreferenciadas y mapeadas. . Los resultados indicaron que, entre 2007 y 2019, hubo cambios significativos en la distribución espacial de los establecimientos de salud públicos y privados: las unidades privadas, originalmente más concentradas que las públicas, mostraron una caída en los indicadores de concentración y pasaron a tener una distribución espacial más parecido a los establecimientos públicos. Las estimaciones también sugieren la ocurrencia de una desconcentración de los servicios de salud más complejos, hecho que es consistente con el proceso de internalización de la educación superior en el área de la salud, asociado a mejoras en la infraestructura de transporte costero-interno (Agreste) del estado, además de incrementar los ingresos familiares. Así, los resultados pueden ayudar en la formulación y aplicación de políticas públicas dirigidas a mejorar el acceso a la salud e intervenir en la mejora de las redes de demanda y oferta de servicios. Es fundamental, en este sentido, basarse en la movilidad espacial de los consumidores y profesionales de la salud, apoyando mejores condiciones de trabajo para estos profesionales.L'article présente une analyse de la dynamique distributive des établissements de santé, dans l'état de Pernambuco, dans les années 2007 et 2019. A partir des adresses répertoriées dans le Registre National des Etablissements de Santé (CNES), toutes les entreprises respectives ont été dûment géoréférencées et cartographiées. . Les résultats ont indiqué qu'entre 2007 et 2019, il y a eu des changements significatifs dans la répartition spatiale des formations sanitaires privées et publiques : les unités privées, à l'origine plus concentrées que les publiques, ont montré une baisse des indicateurs de concentration et ont commencé à avoir une distribution spatiale plus proche des établissements publics. Les estimations suggèrent également l'apparition d'une déconcentration des services de santé les plus complexes, ce qui est cohérent avec le processus d'internalisation de l'enseignement supérieur dans le domaine de la santé, associé à l'amélioration des infrastructures de transport littoral-intérieur (Agreste) de l'État, en plus d'augmenter le revenu familial. Ainsi, les résultats peuvent aider à la formulation et à l'application de politiques publiques visant à améliorer l'accès à la santé et à intervenir dans l'amélioration des réseaux de demande et d'offre de services. Il est essentiel, à cet égard, de s'appuyer sur la mobilité spatiale des consommateurs et des professionnels de santé, en soutenant de meilleures conditions de travail pour ces professionnels. Este artigo apresenta uma análise das mudanças na dinâmica da distribuição espacial dos estabelecimentos de Saúde no estado de Pernambuco, nos anos de 2007 e 2019. Os dados das empresas foram coletados, georreferenciados e mapeados a partir dos endereços elencados no Cadastro Nacional dos Estabelecimentos de Saúde (CNES). Os resultados indicaram que do ano de 2007 para o de 2019 houve mudanças significativas na distribuição espacial dos estabelecimentos de saúde dos setores privados e públicos: as unidades privadas, originalmente mais concentradas do que as públicas, apresentaram uma queda nos indicadores de concentração e passaram a ter uma distribuição espacial mais similar aos estabelecimentos públicos. As estimativas também sugerem a ocorrência de uma desconcentração dos serviços mais complexos de saúde, fato este que é condizente com o processo de interiorização do Ensino Superior na área de Saúde, associado às melhorias na infraestrutura de transporte litoral-interior (Agreste) do estado, além do aumento da renda familiar nas cidades de médio e pequeno porte. Destarte, os resultados podem auxiliar na formulação e aplicação de políticas públicas que visem melhorias no acesso à saúde e intervenham no aprimoramento das redes de demanda e oferta de serviços.
