MORE MEDICAL PROGRAM: FROM PROGRAMMATIC LOGIC TO INSTITUTIONAL IMPLEMENTATION ARRANGEMENTS

Resumo: Among the actions adopted to strengthen primary health care, the More Doctors Program (PMM) stands out, which is an intersectoral initiative and includes a series of measures to strengthen and consolidate Primary Health Care. However, the expectation of efficiency and good performance of the PMM rests on the success of its programmatic implementation. Thus, this study aims to understand what the PPM is like in its theoretical-pragmatic conception and to analyze its implementation at the local level, from the perspective of institutional arrangements. For this, a documentary analysis was carried out to rescue the PMM theory, together with semi-structured interviews with the actors responsible for the Program at the local level, aiming to understand its execution. Based on the analyzes carried out in relation to the emergency provision, it was noticed that the PMM, being a central-level policy, is implemented according to its theory, but reveals a lack of local autonomy to adapt it to local needs. The results reveal a top-down policy, which has, among its main side effects, limitations in the communication process, especially in the policy implementation phase. This exposes the supporting role of the local administration, limiting it in the organization of the health professional's relationship with local needs.

This exposes the supporting role of the local administration, limiting it in the organization of the health professional's relationship with local needs.Dentre as ações adotadas para o fortalecimento da atenção básica de saúde, destaca-se o Programa Mais Médicos (PMM), que consiste em uma iniciativa intersetorial que contempla uma série de medidas para o fortalecimento e consolidação da atenção básica. A expectativa de eficiência e bom desempenho do programa reside sobre o sucesso de sua lógica programática de implementação. Assim, esse estudo teve como objetivo central, entender como é o PMM em sua concepção teórico-pragmática e analisar a sua implementação no nível local, a partir da perspectiva de arranjos institucionais. Para isso, foi realizada uma análise documental para o resgate da teoria do PMM, em conjunto com entrevistas semiestruturadas com atores responsáveis pelo Programa no nível local, visando compreender a prática de execução. A partir das análises realizadas para o eixo de provimento emergencial, percebeu-se que o PMM, por ser uma política centralizada, é implementado de acordo com sua teoria, porém revela a falta de autonomia local para adaptá-lo às necessidades locais. Os resultados revelam uma política top-down, que tem entre os principais efeitos colaterais, as limitações no processo de comunicação, especialmente na fase de implementação da política.
