Resumo: | The present study aims to characterize the emergence and evolution of leprosy in Palmas, capital of the state of Tocantins, until the year 2000, when the first cases were monitored, and from 2001 to 2018, already in an environment with hyperendemicity. It is an epidemiological analysis, anchored in the systematization of publicly available secondary data, followed by a qualitative examination, both about the emergence and evolution of the endemic in that capital. In Palmas, as well as in the Tocantins, leprosy has hyper-endemic indexes. The state of Tocantins is the newest Brazilian federative unit, established, together with its capital, in 1989, when the division of the state of Goiás and the political emancipation of the former North Goiano, with its insertion in the North region of the country, where the leprosy was already a relevant health problem. Despite the few data up to 2000, rates are evident, aggravated by the flaws in the control policy and by the expressive demographic growth of Palmas, which starts to attract migrant populations from the Tocantins itself and other states of the federation, especially from the Northeast, Midwest regions and North, which had the highest leprosy rates in the country, contributing to the increase in allochthonous cases in that capital. Such facts resulted in the evolution and maintenance of the leprosy transmission chain in Palmas, conforming to the degree of endemicity of this environment, with persistent transmission since its creation, despite the therapeutic cure protocol, with public access. It is, therefore, the social production of the endemic, which persists as a serious public health problem in Palmas and Tocantins. |

Trata-se de uma análise epidemiológica, ancorada na sistematização de dados secundários publicamente disponíveis, seguida de exame qualitativo, tanto com relação ao surgimento como à evolução da endemia nessa capital. Em Palmas, bem como no Tocantins, a hanseníase apresenta índices hiperendêmicos. O estado do Tocantins é a mais nova unidade federativa brasileira, estabelecida, juntamente com a sua capital, em 1989, quando da divisão do estado de Goiás e da emancipação política do antigo Norte Goiano, com sua inserção na região Norte do país, onde a hanseníase já era relevante problema de saúde. Apesar dos poucos dados até 2000, evidenciam-se índices agravados pelas falhas na política de controle e pelo expressivo crescimento demográfico de Palmas, que passa a atrair populações migrantes do próprio Tocantins e de outros estados da federação, sobretudo das regiões Nordeste, Centro-Oeste e Norte, que apresentavam os maiores índices de hanseníase no país, contribuindo para o aumento dos casos alóctone nessa capital. Tais fatos resultaram na evolução e na manutenção da cadeia de transmissão da hanseníase em Palmas, conformando o grau de endemicidade desse ambiente, com transmissão persistente desde a sua criação, apesar do protocolo terapêutico de cura, com acesso público.
